Need help going from bottle back to breast at 3 months

My partner and I are struggling trying to get our little girl solely back to the breast. At 4 weeks we were told by our midwives to supplement with expressed breast milk. Till then the baby was only breast fed. Because she wasn't gaining enough weight we were advised she needed the extra calories. Now after 3 months she is mostly bottle fed with pumped milk. This has been very rough on my partner who finds pumping exhausting and very inconvenient. Has anyone been successful at getting their 3 month old to switch from the bottle back to the breast? We have tried nipple shields, finger feeding, lactation aid tubes all to no avail. She doesn't let these get anywhere near her without crying. I feel we need a schedule of removing the bottle slowly and increasing the breast but not sure how to do this without her losing weight. She is a 3 percentile baby so can't afford to lose much weight. She does latch still although is fussy at the breast. Can anyone offer some hope as to how to transition back to the breast only. We have seen some lactation consultants who have been great at helping us get the baby to latch but have offered no help in transitioning away from the bottle. Thanks for any suggestions!!

I have a friend who managed this - she is a peer supporter at the breast feeding support group I go to - I will ask her and report back if no-one else replies :)

Yes we do keep a log of her feeds. We also have a scale at home so we can monitor her weight.

I spoke to my friend and she didn't have much to offer than she offered the breast first each feed and gave less supplement after each feed gradually reducing it to nothing. Her son's weight gain slowed during the process but picked up again afterwards... no magic advice sorry....
