I'm having a nightmare with my 3 week old. I didn't get to hold her for 24 hours after she was born as she was in neonatal due to a few complications during birth. She was a day old before we attempted breast feeding and she was struggling to latch on and I wasn't producing much milk. After 48 hours of her barely having eaten anything, we decided to give her some formula just to get some food in her. We came home after 4 days and have persevered with bf but it's still not coming easily.

Initially baby was falling asleep after a couple of minutes so I was having to express and was not getting a full feed so topping up with formula. I only started to produce proper milk around 2 weeks and still was not getting very much. Now the fact that she's had a bottle so soon has made the issues with latching on properly worse although she does get milk because I can hear her swallowing but it's sooo painful. I've tried to get it sorted a couple of times and have abstained from expressing or formula but it's not really worked as she's a really hungry baby and just cried for more all the time. However in the last few days, my milk supply seems to have finally come through (which has had the unfortunate effect of leaving all my bras, nightclothes and bedding smelling of sour milk as the breast pads I have are not very effective). I've only fed her on the breast for the last 24 hours and she's actually had a full feed and slept afterwards for a few hours each time. Now I've got different problems. My mum did warn me that initially breastfeeding would feel like razor blades on your nipples but that it would pass, but it's only getting worse and now they're bleeding and I've had to give her some formula this evening just to give them a break!

With all the problems, she was a bit slow putting on weight at first but now because we've been topping up with formula, she is getting on fine and is really filling out, but in an ideal world I'd rather not have to spend the money on formula and would rather she got the natural goodness of breast milk. She is sick less with breast milk (even when it's expressed and out of the bottle so it's definitely the milk and not just the method of feeding) and on a selfish note it will help me lose some of the baby weight.

Because I've been quite sore down below waiting for stitches, bruising etc to heal up, I've not been out of the house much but I'm hoping to go to my local bf clinic this week to get some advice on how to get her to latch on properly as I think that is a big part of the problem. It's not until Friday though and I'd really like some advice in the meantime.

It sounds as though you've had a tough start! Well done you for persevering though - you're doing really well. Have you got some Lansinoh cream for the sore nipples? It's expensive but it works. I also found bathing the cracked nipple in warm salt water after each feed helped.

Have you got the info on positioning to get a good latch? When my twins were newborn I found the biological nurturing positions worked well - lots if info if you google. Basically the baby roots to find its own latch, and you get to lie down, which is a bonus!

Can't offer much in the way of helpful advise. BUT can give you a bit of happy news.

I had similar problems and DD fed fine once we got them sorted.

You need to get someone to look at her latch, if you can't get someone to come to you, I found some pretty good stuff on YouTube!

Yes, you really need someone to have a look at the attachment so it's good that you are going to go to a group on Friday.

Maybe it would help to go back to basics- check that her mouth is really wide when you bring her to the breast,
keep her head, neck and body in line
keep her body close in to yours
lead with the chin so that she has more of your areola showing above the nipple than below it.

Some women talk about the pain feeling like razor blades when they have thrush- it might be worth ruling this out though it's often misdiagnosed and is usually down to poor positioning and attachment. Lots of good stuff on the Breastfeeding Network website on this.

Lansinoh is good for soothing and helping to heal any cracks, but take not to use too much or your baby will slip off the nipple.

I'm guessing that you are not under mw care any more but maybe you could seek some help from the HV if they have good breastfeeding knowledge.

Well done you! You are doing brilliantly to be breastfeeding after that start. I also topped up in the first few days as baby was struggling and my milk took what seemed like ages to come in. My baby lost around 10% and the midwife marched us back to hospital, so well done for keeping baby fed and getting your milk in properly.

Are you doing the nose to nipple thing? I found it a huge help when I did it in a disciplined way. I also got on well with lying beside baby, but still needed help positioning her nose to nipple. I also rang some of the bf helplines (la leche etc) and they are really helpful. I can also vouch for lansinoh cream, and recommend their breast pads. They are fab.

Thanks for the advice. I got some sachets of lansinoh cream from midwife in hospital but hadn't used it much as I forgot I had it when I came home. I've also tried a different bottle and teat shape to see if that makes a difference. She's not opening her mouth wide enough and is suckling as she does on the Tommee Tippee bottles we have been using. I changed to Medela today which is the system I'm using with the breast pump anyway so I'll see if that makes a difference as she opens her mouth wider with them. I'll also take a look at the biological nurturing website to see if that helps. I did try lying down beside her when I was in hospital as I was in too much discomfort to sit for long periods so I'll see if it hurts less.

Have no advice (apart from the Lansinoh) but wanted to say you have done amazingly well - hope you can get the right advice to just sort the pain quickly.
