Help, anyone around for some quick nipple treatment advice?

I have a white (milk filled?) spot that's just appeared on my nipple tip, just noticed at end of BF session. Can someone talk me through what this might be and short-term do's/don'ts?

Already had so many problems with BF and could do without another wrong move with icky consequences. At the weekend...

Basically, do I feed that side or not? Don't want to cause more damage.

Feed on that side lots and in different positions. Soften it before feeding with a warm flannel.

somettimes it comes down to popping the bugger in a sterile way.

Brief history, currently being treated for duct Thrush and just over mastitis. It's been one thing after another and I'm normally so much more healthy.

Agree with Pesto I had a milk blister (it's just a blocked duct so milk can't get out) and it quickly turned to agony. I had this rock hard section of boob that didn't go down so on the advice of mumsnet I popped it with a sterile needle.

Yes.
Basically skin has grown over the end of a milk duct and a little bit of clogged up and/or grainy milk is now blocking it and will cause milk to back up in your breast. Can lead to mastitis. Same principle as mastitis, you need to get the milk shifted.

Sorry to hear you've had such a rubbish time Sad. Have you had any RL support?

I boiled the needle for a few minutes first. I'd try and feed it out first if you can but if it gets painful pop it. It didn't really hurt to pop it and it was a big relief to express afterwards.

Yes, have had good support, but weekdays only. These things have a nasty habit of cropping up at weekend. Will try some feeding before the needle, I think.

You might find taking some acidophillus useful in counteracting the effects of the anti biotics (for mastitis I'm assuming), otherwise you may find the thrush returns.

Pleb/milk blister - agony, get rid asap. Heat, rubbing with flannel, feeding all suggested but needle was the only way for me in the end.

I had the doctor pop it as I didn;t know I could do it myself. The force of the backed up milk hit her in the face. Blush

She gave me my own needles so I could so it myself the next time...

I've had one of those. BF clinic lady said to rub it with a flannel or muslin (cringe) before feeding. It definitely helped and might be less toe curling than the needle....

Thanks so much guys, my trusty 8-week old baby shark has taken care of it. Phew. Just when we'd managed to fix the latch on that side.

Is there anything helpful I need to do now to heal/stop infection?

Getting bigger could be due to milk being drawn into it and unable to escape. It may go down a little again after the feed. You really do need to shift it, otherwise your duct will start to get inflamed and you'll get this feling of tenderness tracking back in a wiggly line through your boob Sad

When the stuff does come out it might be gritty, toothpasty, yellowish (like a squeezed spot) etc. There may also be more than you expect from further back in the duct, depending on how long it's been blocked.

Feeding leaning forward, or dangling boob in bowl of warm (not too hot!) water might help -i.e. gravity.

Some people wander round with a ball of cotton woll soaked in olive oin in their bra to try to soften the skin before the next feed.

With the needle thing... it's not strictly advised to do it at home because of the infection risk (though arguably a boiled needle is less of a risk than a fingernail). If you do read up and decide to give it a whirl, then you only need to make a tiny prick in the skin in the middle, not jab it in deep or anything . People I know who tried it say they barely felt it, and afterwards were able to express the gunk.

Sometimes the blasted things seal over again, so they can go up and down a bit.

Also, if there's a cause of this (and maybe your mastitis too?) it would be good to try to work that out. Both result from less than ideal drainage of the breast and often suggest there's something that can be tweaked about the way the baby feeds.

Keep feeding on that side, if you can in various positions. If you start to feel at all full express (by hand), and consider doing this after feeds anyway for a short time.

Keeping the milk moving should prevent infection, cos the milk is antibacterial. The cause is usually a simple blockage, not any kind of bad germs or anything.

Thanks Pesto, so helpful. I'm still trying to get to bottom of all manner of problems on right side, while left side has been relatively normal since week 2. I may have to pick your brains in future as it sounds like you've unfortunately dealt with some similar bad boob behaviour.
