Supa I'll give you the advice the BF specialist gave me today, and if any of it works for you, then great!
Did they check for a high-pallate or tongue-tie when baby-supa was born? These issues can both delay BF.
RE: nipple shields, measure your nipple side-to-side (how far it sticks out is irrelevant apparently). I ended up needing the large ones...not standard and have gone for the medela large...which the BF specialist was very pleased with (one thing the hospital did get right) as the medela ones give you very good sensation and stick quite well to the breast (so they're not sliding about and thus loosing latch)!

Couldn't find them in mothercare etc. but these ones are doing me great...apparently! 
A lot of the problems women have with pain etc (especially if you feel you have 'flat' nipples) is due to there not being enough nipple in the mouth.
You know when you're expressing, the sheer quantity in the pump (literally all your areola), that's the amount that shoud be in baby's gob...pincering it in, like a hamburger's a good way, though BF specialist suggested using a finger to guide the whole thing in nice and deep...support under boob with outer 3 fingers and point in with the pointing finger until there's a decent latch, and then remove the finger, use the thumb to support/guide the upper breast.
Also, I've been positioning her wrongly. I'm doing rugby-ball at the moment and my nipple needs to be pointing AWAY from her and over her nose...that way when she's opening wide, you're getting enough nipple in at the bottom. She was doing well at sucking in the top herself.
When she's latched her chin should be massaging the breast and she should be nice and close and "tucked in" under the breast....I'd been putting her too much on the side previously...assuming the size of my boob was rahter intimidating...apparently similarly to when 'front feeding'...nice and close and tucked in/under.
Snufflix is a bit furious at points, and apparently I just have to be a bit more forceful. If needs be, start with a shield, de-latch after a few minutes and try without. If not, then whack if back on. Baby-steps...try it and if it's not workign now, go back to it later. She's a willful little sod and basically I have to let her know who's boss and force the issue....she's exactly like this with bottles too, but it's somehow easier to be forceful with a a bottle.
Other kit: feather pillow (covered with a waterproof cover etc) as they're eaier to lound to baby and a hand-towel to roll-up (you can then use this to steady the head and neck, so you can to 'hands-free').
Keep looking for support. It's a shame you've not found it/it's not available in lincs...hopefully I can keep it up. It really was very satisfying to see her latch properly.
