Severe mastitis for 14 days now and no relief

Hi everyone,

I am just hoping to hear from someone else who might have experienced something like this

I have had mastitis for two weeks now - for the first 5 days (Tues-Sat) I tried to reduce the lump myself with warm compresses, lots of feeding, different nursing positions, warm baths, hand expressing, cabbage leaves, combing etc

On the Sunday morning I was in agony, so went to the out of hours doctor at the local hospital and received Amoxicillin antibiotic. I took a double dose on Sunday (8 tablets) and then 4 tablets a day until the Thursday, when I visited my local GP as there was no improvement (but no worsening at this stage)
He told me to stop taking the Amoxicillin and changed my prescription to Keflex.

The following day, Friday (10 days) I went back to the out of hours doctor at the hospital in agony. He was annoyed that the GP had stopped the Amoxicillin, took me off the Keflex and started me on 4 tablets of Flucloxicillin a day (all of these have been different forms of antibiotic, just to make clear). He reassured me it was not an abscess.

Monday (yesterday) I went to my GP surgery (but saw a new GP) who checked me again, as I was in a lot of pain - he confirmed that it was not an abscess and told me just to carry on with the Flucloxicillin. He asked me to come back on Friday for another 7 day course of the same drug. He also told me to take codeine & paracetmol for the pain.

Today I'm in just as much pain, even with some additional pain relief...the lump is very pronounced and very painful - even to put a soft cotton sleep bra on the breast is intensely painful.
I cannot hand express as I cannot bear to touch my breast. My baby will no longer latch on, unless coaxed from the other breast after the initial let down.
The skin is starting to split and peel on the lump (not on the nipple, although the lump is right beside the nipple). Also, where the skin is peeling, there is a dark spot like there is a little blood under the next layer of skin (but it is not bleeding on the surface, IYSWIM?)

I'm at a loss. I know that after 4 doctor appointments in 2 weeks that it is not (yet) an abscess, but the pain is huge and the antibiotics don't seem to be having any effect. Perhaps it is because they kept changing the type I was on? I know different doctors have different preferences.

I feel like I'm waiting for it to become an abscess, just so they can treat me and relieve the pain. Which seems so wrong!

Does anyone have any similar experiences? I don't know what to do with myself. I'm only just coping as my husband is at home at the moment, but he is going overseas on a business trip tomorrow and I don't know how I'll cope....I can only hold my 6 month old for a few minutes before the pain gets too much, and he's screaming with the pain of 4 teeth coming in at once (he has 4 already, and I can see the next four white in his gums!)

I don't know what to do, and I don't know what I'm going to do for the next two weeks of being the only parent at home

ARGHHH! help me, please x

First of all -poor you. Wanted to respond. Now I need to read in detail.

NonnoMum · 17/05/2011 23:17

Sorry - I don't understand. Is this a new-born or a six month old? Bit confused from your post...
Also - look after yourself. Paracetamol to bring your temp down. Think you might need to find a breastfeeding specialist for advice rather then your GP. And NEVER apologise for how many appointments you need to get a health issue sorted. It's everyone's interest for you to be WELL...
Good luck. Hope some better advice comes along soon.

Right, A&E tomorrow I'm afraid :( it sounds to me like you have an abscess I'm afraid.

You should have been given flucloxacillin in the first instance, amoxillicin is useless for mastitis.

I hope that you get some relief soon. I really feel for you. I have not had an abscess (but my fair share of mastitis) but I understand that when the abscess is aspirated you will start to feel better immediately.

Let us know how you get on.

bigpinkellie · 17/05/2011 23:21

Sorry for any confusion, this is my first baby, age 6 mths

First experience of mastitis also :(

Thanks for your reply, I think I might have to revisit doc again tomorrow..

Actually, agree with poster who suggests A and E. Sometimes getting someone to look at your health problem right here and now is a good choice.

PenguinArmy · 18/05/2011 03:53

I would have thought by now you'd be on IV antibiotics so agree with AnE option.

bigpinkellie · 18/05/2011 07:10

Update: woke up 30 mins ago, looked in my bra and where the skin on the centre of the lump had started to split & peel I was suddenly gushing a lot of green pus and blood. It literally looks like a little rip. Called the out of hours doctor, and he just said my abscess had burst (thanks to all those doctors who scalded me for a mumsnet self-diagnosis!!) and I'm to go to the nurse later for a dressing and keep up with the antibiotics. But I know there's more infection/pus in there so I feel a bit fobbed off. shall I insist on having it drained further do you think?

Sorry that it turned out like this.

I think you need to forget about your GP and out of hours doc and go to A&E where they know what treatment you will need. Agree you may need IV antibiotics and to have it drained.

conhill · 18/05/2011 08:49

I would recommend the A&E option also. I was in a similar situation to you, many GP visits, different antibiotics. After 4 weeks of this (though mine didn't burst) I went to see our BF advisor who told me to go to A&E straightaway - and they put me on IV antibiotics. You really need to see a breast specialist for this, and the A&E route will be quickest way to see someone. good luck

Sorry to hear what you are going through. Agree, think it's time for A%E now.

bigpinkellie · 18/05/2011 08:57

Just got back from GP (who I only got to see because I cried at the reception desk!) - yet another GP I've not met before.

She cleaned it up and dressed it, prescribed a further 7 days of Flucloxicillin and is referring me to the breast clinic at the local hospital. She said I can expect a phone call or letter for an appointment at the hospital, to see how large the abscess cavity is.......I have no idea of timescale though. She said she was referring me urgently to two hospitals but she had no idea what their availability is like so I still have to wait to see if either have space for me. She said that A&E would be pointless now as I have been referred urgently by her.

God, I am so so sick of this - I was doing so well with breastfeeding for 6 months, was just starting to wind down and this came totally out of the blue. My husband is rubbish at sympathy or help. He'll hold DS but only for 30mins if I'm lucky. He actually said to me yesterday "Honey, it's not the end of the world now, is it?"

ARGH!

how's you today? only just saw thread. how poor of your GPs....
anyway hope you ok - what a dreadful fortnight you've had Sad

bigpinkellie · 22/05/2011 20:52

I'm doing OK thanks, the pain is much less now the abscess has ruptured

Thanks to everyone for the advice, it's much appreciated

I'm going on Tuesday to have the affected breast scanned, and then I'll be referred to a surgeon to have the damaged area removed/cleaned out and then stitched

I'm really worried about weaning DS off the breast now - we'd only cut back the tiniest bit and I got hit with this, so I'm slightly worried about how I might get mastitis again

Thanks again everyone xx

Oh you poor thing, gosh I hope things go well for you on Tuesday.

I have no advice on weaning I'm afraid. Perhaps tiktok might be able to help.
