How do i get the second breast into our BF routine??

Please help with how I can re-introduce my second breast in our BF routine!

I'll TRY to keep this short:
13 weeks old DS has had difficulties since birth to latch on to one of my breasts which has an inverted nipple (I call this for simplicity sake the "bad breast ;)...the whole process upset him so much that I ended up feeding him for 3 months only on the "good" breast, whilst keeping some milk production going on the bad one by expressing - always hoping that he would latch on to both at some point
He is a cheerful, content baby and looks healthy to me, however, the HV flagged last week that we need to keep an eye on his weight gain which has slowed down over the past weeks (he was 3.2 kg / 25th centile at birth and has now been on the 2nd centile for the last 4 weeks (weighing 4.8kg at 12 weeks)
Feeds usually take quite long (min. 45 minutes), as he spends more time on happily sucking away and only interrupts this with a couple of pretty short (3 - 6 minutes) feeding intervals-didnt mind this as most important to me was that he was relaxed and happy during his feeds
A couple of days ago he was suddenly able to latch on to the bad breast. I was initially very excited about this but am now wondering how to include that breast into our BF routine and to ensure that he gets enough!! Main problem at the moment (I think!) is that milk production is currently lower in that breast, so I guess I cannot simply swap breasts with every feed. Whenever I now start the feed with the bad breast, he is not very happy, sucking like mad and (again I think!) is a bit panicky about the current lower milk supply. I then swap after ca 10 minutes to the good breast and he guzzles away like his life depends on it - followed by several rounds of spitting a lot of it up again!!

In view of his not too great weight gain I am now concerned whether I should continue trying to feed on both breasts at all and if so, what I need to do to do this correctly. Sorry this turned out to be a pretty long story but wanted to get the facts together! Any advice highly appreciated!!

what happens if you offer the "bad" breast at the end of a feed?

Cosmosis - thanks for taking the time to reading my rather long post!! If offered at the end, he is a lot calmer, takes in some milk ( I think he would rather like to just use it as a dummy though...) in any case again he is spitting up quite a lot after the feed

Have you tried swapping over mid feed, so he's filled up a bit and isn't so desperate, but still quite hungry?

I think you might be best off ringing one of the helplines tbh for soem proper discussion, do you have the numbers?

Have you seen a BFC or clinic (so not just normal MWs/HVs) about your latch & positioning? 45minutes+ for a normal feed seems quite a lot at 13 weeks TBH so given the poor weight gain as well, I would be concerned that your LO may not be feeding efficiently (they need a deep latch to feed efficiently).

Afaik, supply should be fine because your "good" breast will have adapted to provide what he needs, so as long as you're not only offering the "bad" breast at some feeds, he should be getting ho wmuch he wants.

Sicking up a lot may be a sign of something else going on too...reflux or an intolerance perhaps.

Japhrimel - many thanks. I have seen a BFC when he was only a couple weeks old as already then he was feeding for ca 45 mts. Latch/positioning was fine and the comment was that he is just taking his time...Regarding the increased amount of sicking up, this is only happening since we are using the second breast so Im hoping it is not reflux / intolerance related.
