Advice on ebf baby with trapped wind/constipation

DS3 is 14 weeks and ebf. Every now and then he will go a day or two without having a poo and during this time will be in terrible pain. He brings up his knees and sobs uncontrolably. On occassions he refuses to feed and has gone up to 10 hours in the day before having a feed. As soon as he has pooed he is back to his usual happy self. I have spoken to HV about this who have suggested offering water and have shown me some massage techniques to use on his stomach.

Yesterday he was so upset and in such pain. He hadn't pooed for 2 days and I couldn't do anything to ease his discomfort. Spoke to HV again who said he wasn't constipated unless the poo was like rabbit tods when it finally comes out. It isn't it's slightly thicker than usual but still looks like normal baby poo. She didn't seem overly concerned but I am finding it extremely distressing to see him in this state.

I have made an appointment with GP whilst waiting to speak to HV as I am very concerned. I cannot pin point anything in my diet which triggers this to happen and wondered if anyone has any advice.

Today he's has finally had a poo (2 infact) yet when my sister was holding him she could feel his stomach churning. Surely this isn't right?

Have breast fed both his brothers and never experienced this.

My cousin has this with her DS and was told by HV to give prune juice. Not sure if this is good advice though....

I did wonder about that but thought he would be sick if he was lactose intolerant. I'll give it a try. The problem is he can go a couple of days even weeks without it flairing up so might be difficult to pin point.
