12 months old screaming after night feed

I will be seeking medical advice but I am hoping someone has some insight or experience with the same issue.
My nearly 12 month old has always been EBF but is eating brilliantly and now down to just one feed before bed.
However he has suddenly started screaming inconsolably for the last couple of nights immediately after unlatching. He takes a while to calm down and will eventually fall asleep after rocking in the pram.
I am wondering if perhaps this is teething or maybe the start of an ear infection from lying on his side while feeding. I know he is tired and it's almost as if he wants to sleep but can't fall asleep himself.
Does anyone have any ideas?

Could he be overtired OP? Have you tried moving the feed a little earlier?

I think this is probably the main cause as he has just started full days at nursery. We are doing his normal routine earlier as he seems more tired but perhaps it's an idea bring it forward even earlier. Just awful to see him in such a state :(

Sleep regression? My LG had a sleep regression around this age and it was torture. Screaming the place down until she tired herself out. It lasted weeks. I feel for you x

Does he have any daytime naps? If so, how does he fall asleep for those? 12 months is kinda young to be only having one milk feed a day, or is he having milk from a cup as well? (Breast) milk is normally still a significant source of calories at this age, "food is for fun, until you're one" and all that.

It's probably that he's adjusting to his new routine at nursery, and maybe isn't getting as much sleep in the day as he did at home. As suggested above, maybe bring his bedtime forward so he's not as overtired.

My son went through something similar, and it was due to overstimulation and overtiredness. As well as adjusting the timings of last feed/bedtime, think about how you're backgrounding it too - lower the lights earlier, reduce sensory input so quieter voices/music and tv off earlier, make that final feed even more connected and bonded than usual and aim to get him off to sleep on the boob for a while. Nursery is a LOT and the time you process stuff is before sleep - have you ever been really tired but then start going over everything that's happened during the day?
