Our 21mth old son is usually good at bedtimes. He has his routine and settles pretty quick normally.

Starting monday he just did not want to go to bed. My husband tried soothing him, then leaving him to cry and going back to reassure him, but he just got worse and worse, till eventually he made himself sick with screaming.

Since then each night he cries The minute his dad even makes a move to put him in his cot and gets himself into a state making until he's sick.

If he has a paddy in the day i ignore it till he calms down, but can't ignore it when he has been sick on himself / bed / floor.

I just feel so useless and don't know how to nip this in the bud.

My DS used to do this and sadly you really do have to ignore it as best you can. It can't 'work' IYSWIM.
YOu have to clear the mess up but just do that with as little fuss as possible and put him back down.
I would also try and change the bedtime routine a bit so that he doesn't get the usual triggers.
Poor you - it was horrible . hope it doesn't last long

My 20 mth old has started having the most almighty paddies when put in his cot (not making himself sick thankfully). I got out the old disney light projecter (that DD had as a baby) and that seems to shut him up!
