ADD/ADHD 4.5 year old?

My Daughter is 4 years 5 months and started Reception in September, she is one of the youngest in her class.
I was concerned with her lack of progress with reading compared to the other children in her year and posted about it on here, but was reassured that she is just young and perfectly normal by the other mumsnet members.
I had her first parents evening last week and her teacher told me that she was below the age related expectations in reading and maths which I was expecting to hear tbh.
Her teacher said she had settled in well, gets on with the other children is very creative and imaginative with her role play, however she struggles to follow basic instructions when she is in a big group (carpet/circle time) and gets distracted easily (eg 'distracted by fluff on the carpet when all the children are sat learning together). She is also having what they call 'intervention' with 4 other children in her class, whereby she will sit with the whole class and learn phase 3 phonics but then go off with the other children with 'intervention' and go over and over it again including phase 2 phonics. Apparently she does well in smaller groups.
She can recognise 90% of her letters and is now starting to read books by sounding the letters out but she still hasn't grasped blending yet (most of her peers can read books now).
She also gets distracted and fidgets at home when she is eating her tea or doing homework with me.
Her teacher mentioned 'barriers to learning' and getting her assessed by SEND (?) next year (she said it's too early to tell yet). She didn't specifically mention ADHD, but I'm wondering if that's what it may be.
Any advice would be much appreciated.

It’s great that the teacher is so proactive! I think it’s definitely worth assessing for ADHD and ASD.
