4 year olds behaviour

My 4 year olds behaviour is quite bad. He's always had problems with eating and sleeping he's quite a fussy eater and won't try anything new, he sleeps very little and only sleeps a few hours through the night. Behaviour is the same at home and nursery and that's when we started to realise it was becoming a problem. He has lack of understanding like you have to speak in short sentences otherwise he would just drift off and focus on something else. He also has a lack of understanding around dangers like roads and strangers things like that. He has suffered from loose bowels for the ages of about 2 years old which was put down to TDS but nothing's changed, he's still in nappies and is showing no sign of wanting to potty train at all. Referrals have gone in for speech and language and also peadiatracian and I'm just waiting for appointments coming through. I just need some advice until he's seen

Make an appointment for a longer chat with Nursery. Ask for their advice. They will know him and will be able to suggest strategies.
DO they use visual cues for him ? (either photos or symbols or objects of reference ?) if so, you could use them at home too.
Say his name first. make sure he knows you are talking to him, then use single words or short phrases with key words.
"ds, coat" rather than "Come on now ds it is time to put your coat on, we're going out and it is raining outside" etc etc.

Tried all the techniques they use he's doesnt listen he either has a massive tantrum or just laughs in your face. They cant handle him at all

Then perhaps they could get some advice from the Area Inclusion Officer or Area SENCo or whatever support they are able to access in your area?

could it be autism, I see quite a few red flags from your description... lack of understanding at four years old is very concerning when you consider a typical 18 month old has a receptive language good enough to follow simple commands while a two year old is able to follow much more complex two step commands. How is his speech? and non verbal gestures?

Speech has just recently gotton better few months ago there was only family that could understand what he was saying

he does sound quite delayed in both expressive and receptive language - He should be compensating by an increase use of gestures (as a speech delayed infant would typically do) like a marked increase in pointing, showing, giving, reaching, eye contact, etc. in order to communicate.

He is more vocal these days but he does still point to what he wants

Does he point to share interests? like point to a plane in the sky, etc?

D's has been awake since 8am it's now 5am am exhausted he's been crying for hours over a slight bump to the mouth ( nothing major did while having a tantrum) and won't go back to sleep
