18 month-very little speech.

My ds is 18 months old. He has two older loud brothers, who take over a lot of the time! He is not saying much at all though, very rarely says mama(usually when crying but not sure if it even is mama or just a sound he's making!), he sometimes says something that sounds similar to 'thank you' when I give him something, and has said 'more' a few times too. But other than that he just points to things or kind of shouts at me to get my attention and drags me to what he wants! He understands everything. At what point should I contact nurse? They always over-react so don't want to make it an issue if he's ok. He is 18 months but looks about 2.5 so I don't know if that is makin me over-react.

A bit young to worry seeing as he is communicating well by your account. Youngest with lots of siblings so where is the incentive for him to start chatting when he is able to get his message across. There is a standard 2-2.5year check up with the HV so wait until then to check milestones.

I think 18 months is very little tbh and wouldn't worry yet. I agree with pp suggesting to review at 2.5. My DC1 had very little speech at 18 months and wasn't great at 2. Then it was like someone flicked a switch on his second birthday. We had new words every week and by the time he was nearly three he was speaking in sentences and we couldn't shut him up! If he communicates in other ways, pointing, shaking his head etc i really wouldn't be concerned at this stage.

Agree 18 months seems a bit young to worry, especially given his older brothers! Can you remember when they really started talking?
