Digestive issues in 4 month old

Warning - lots of discussion of poo. Sorry!

My DS is 15 weeks old and has seemed to suffer with digestive issues since birth. He's had bad wind and no matter how much I burp him he seems to have a lot of bottom wind. He also seems very sensitive to bowel movements and can wriggle, grunt and squirm for hours before doing a poo. This seems to really frustrate him and he ends up in tears. He also seems to have to suck in order to pass the fart/poo, either on his fists, my hand or my breast. He generally poos 2-4 times a day, which is usually yellow bf poo, sometimes quite runny. However, sometimes he doesn't go for more than 24h. On these occasions he will squirm like he is constipated until he goes, but his poo will be normal. He can also be distressed at times, crying and shouting in the lead up to going. I was hoping things would improve as he got bigger but they haven't yet. In fact things are worse at night. He can often be up a lot of the night wriggling and squirming until he poos in the morning, often going a couple of times before lunch. We are cosleeping as he won't settle in his moses basket so I'm not getting much sleep due to the squirming and my nipples are in shreds as he wants to suck all the time (feeding and then just sucking).

He also has occasions where he is inconsolably crying for about 10/15 mins about half an hour before doing a poo. During these periods we noticed a firm bulge on the right if his groin and so he is now being referred to the hospital to determine if this is a hernia.

I had been thinking that a lot of these issues we're due to his digestive system being immature but now that he is a bit older I worry there is something irritating his tummy or causing him distress. He's exclusively bf and I am already on a dairy free diet due to milk protein intolerance. I try to keep my diet simple and haven't noticed any obvious dietary reactions but I am keeping a food diary.

Does anyone have any experience of symptoms like this? Does it sound like an intolerance? I thought he'd maybe have other symptoms if this was the case? Anyone any other ideas or experience of when things might improve?

Also, re the hernia, I thought these we're painless but his does seem to cause pain at times. Is this normal? It's not constant pain, it does pass but feel so sorry for my little guy. I did wonder if this is causing him some discomfort a lot of the time, leading to all the squirming and grunting etc but not sure how plausible that is.

My DD2 is 12 weeks and to me this sounds very normal and just the same as my DD and my first DD... my Dd can get uncomfortable before doing a poo and also can go 24 hrs withoutpooing, although she only goes once a day. Everything else sounds the ssame as well as the crying etc. When she is particularly uncomfortable I have given her infacol and tried cooled boiled water to help things along, both have had limited sucess but have on occasions seemigly eased things a bit. My DD does not have a hernia though so I don't know if maybe your little one is having added discomfort from that but I hope you get things sorted soon.

Thanks doctor brown bear. Good to know its normal, just so frustrating that he gets woken so often by wind when otherwise he would sleep longer. He has woken every 30 min since going down at 8.30 so far tonight :( and spent all day griping until he finally did a poo at dinner time!

Tbh it sounds v normal for a baby with a food intolerance.... Not normal normal iyswim? Dd and ds were both like this. With ds I made no dietary adjustments as I was assured he was 'normal' by docs and hvs and he was an appalling sleeper for 16 months (who also threw up everytime he ate dairy when starting solids). With dd she had alot of paeds input as she was prem I was told to ditch dairy and soya and she slept! About a third of cmpi babies also react to soya so it would be worth eliminating that for a month to see if the situation improves. If not check religiously for milk traces and if that turns up a blank keep a food diary to see if you can spot another culprit

My DS is almost 12 weeks and has been similar since very small - I also have an almost dairy free diet(I eat goat/sheeps cheese) as I am intolerant and also do not eat wheat! He was exclusively bf until last week when we introduced 1 bottle of Hipps organic! DS often squirms and puffs and goes red in face, pulls up his legs and screams in trying to do a wee.
Recently he has become quite clingy and will not settle in his bouncer ( he used to sleep happily in it during the day!) and at times we can't put him down without him crying. Very difficult to get anything done and last night was in tears trying to bath and sort bottle when DH at work all evening. Not sure if this is colic as well though! Don't really want to cut out all soya related stuff as I am already well under weight!
