Lack of bladder/bowel control in 5yo HELP!

My dd1 is almost 5 and started school in September. Up til then she was pretty much in control of her bladder and bowel movements, with the odd accident very occasionally. Since starting school we've had a problem with her having accidents, mainly bowel movements. This normally happens at school, although sometimes at home. We spoke to her teacher who has been asking her to go to the toilet regularly and it improved slightly. Since going back after christmas however the situation has really deteriorated and she's been wetting her pants almost every day, as well as having a bowel movement in her pants yesterday and going in the bath in the evenings twice. She just doesn't seem to know when she needs to go anymore. I spoke to her teacher again this morning, and even though she was reminded constantly about it today and had 4 wees in the toilet she still went in her pants as well.
She has real issues with food and doesn't eat any fruit and vegetables at all so I thought that might have something to do with her bowel issues (ie her stools are very soft and she doesn't get much warning when she needs to go) but that doesn't explain this huge step backwards in bladder control.
Has anyone had experience of this with this sort of age child? I'm so worried for her and I just don't know what to do!!

Is it to do with the toilets themselves? Are they smelly? Hard to use? Do the "big" girls hang about in there? If it's only at school I'd look at that. I'd also get her checked for a UTI.

I can't see why it would be the toilets - they're clean and not smelly and are only for the children in her class to use. I think she definitely needs a check-up at the dr though.

Personally if it were my dd I would go the doc there may be some other medical reason why its happening so you need to eliminate that first before looking at psychological issues

Thanks for your responses. Doctors appointment has been booked now and hopefully that will at least rule out a medical problem.
