How to explain about death/disability - anyone suggest any good books?

My friend's grandmother has just died and her son is asking lots of questions about death and dying.

Can anyone reccommend any books which might help her to explain this to him?

I'm also looking for a bit of help as our dd has a bladder problem and is starting to feel really different to other people. Her younger sister is disabled and I feel it would be good to explore the idea that 'everyone is different'. If anyone can suggest any books which might help with either of these issues, I'd be really grateful.

For disability there's a good book called "I'm special" by Jen Green. I don't know whether it's still in print but it's available used on Amazon (type I'm special in search and it's the one with a girl in a wheelchair holding a ball).
I got it for my dd because she was born missing her hand and there is a teacher in it with only one hand.
