Forename change

In England, if I change my infant forename on my birth certificate within 12 months of birth, is it considered a legal name change? And if asked, “Is this your name since birth?” — what is the correct answer? Especially they had a passport issued before with the old name which they used for travel.

does any goes through this process before to tell me the correct answer as it is confusing a bit.
