When should I have my thyroid levels checked again?

Big family history of thyroid issues (Graves and Hashimotos). I've got high TPO antibodies but previously normal tsh and t4.

Went for a smear test recently and nurse repeated them as I am a bit tired recently and have put on weight.

I've got low ferritin (normal hb) and Vit D and thyroid bloods are..

Tsh 4.24 (0.3-4.4)
T4 12 (9-19.1)

I'm an ex nurse so understand that my thyroid may be starting to give up as my tsh is rising and t4 is at the lower end too. I'm not sure what the management is re repeat testing.

I've got an appointment in a weeks time. I am feeling rather tired but that may be low vit d.

Any advice re questions to ask would be really great!

Given the family history and antibodies it's marginally possible, with symptoms too, they'd possibly consider starting a low dose, even if only 25 alternate days. You could ask them to check with an endocrinologist. Do you know if you have a goitre?

There's a possibility you'll swing back and forth a bit yet though so they're likely to want to see you further over tsh 4.5.

I can't remember how often they test in this situation but 3 - 6 months is reasonable if you can cope.

Vit d can be low with it (they're nit sure why) ferritin too due to how it affects iron metabolism.

Get test done as early in the morning as possible as tsh is slightly higher then.

Look through all the symptoms and take a list in case it helps. Check the nice guidelines too.

I feel for you; thyroid things are always so slow. The British Thyroid Foundation are really good; you could look on their website.

Thanks warrior. I've been sluggish for a while and gained some weight. I'd rather get treated sooner rather than later.

That is essentially a "normal" thyroid function result. I'd not treat based on that but review again in 6-12 months. Even subclinical hypothyroidism the guideline is to repeat.

Your TSH is way too high and shows your thyroid is struggling. Even though that value is (just) in the 'normal' range, it should be in the 1-2 range which is the optimal value. The so-called 'normal' range in the UK goes way too high. Also your T4 value is very low, again showing your thyroid is struggling. With that family history, you should be started on at least 50 mcg of Levothyroxine. I was prescribed Levo when I had a TSH of 4.95 and raised TG and TPO antibodies, like you with a family history of thyroid disease. You need to go back to your GP for further testing and stress your family history. Best of luck! Smile
