Likelihood of low Papp-a and low HCG in next pregnancy

Hi everyone

Last year we were pregnant for the first time and at the 12 week screening my Papp-a came back extremely low (0.05) and my HCG also extremely low (0.06). NT was measured normal. I came back at 1:5 chance of Edwards and Pataus and following more scans they found a cystic hygroma which led us to unfortunately terminating the pregnancy, it was a horrible upsetting time.

Fast forward 6 months later we are overjoyed and surprised to find ourselves pregnant again! I am currently 8wks but I am on pins until the 12 week screening hoping we have a different story this time!!

Im looking for reassurance that what happened won’t likely happen again.. has anyone had an experience like this but then had a totally normally pregnancy with normal screening results the second, third, fourth time around etc?

Just really need a little boost right now..💖

came across your post and thought I’d share my recent experience.
Had an MTOP due to T18 back in 2019. We fell pregnant fairly shortly after and everything was well, it was an anxious time and back then we paid for private NIPT testing as we couldn’t face how prolonged the NHS route was.

fast forward to today, I am 14+4 and my experience has been very positive but it did take a bit of advocating for myself. I read that NIPT is now offered on the NHS if you have a high chance result. I had highlighted to the midwives that I have a history of this and I would like the NIPT to go ahead ASAP as I know I will always be classed as high risk.

what I’ve learnt:

  • the NIPT screening programme has been delayed until April (officially) but I was still able to get one
  • you no longer get classed as high risk with previous instances of syndromes, you just get a low or high result. I was previously told I will always flag as higher risk even if the result is 1 in 250000

I was lucky that I happened to be in an antenatal clinic because they lost my booking bloods and a screening midwife was walking past so I asked her the question, she replied with the above points. Nonetheless she checked with her supervisor and even though she sent me on my way, she called me a few hours later saying she will give me the NIPT

i went in at 10 weeks, came back low risk, had my screening done 2 weeks ago and just received a letter also saying I’m low risk

bit convoluted but I just wanted to reassure you that your feelings and anxiety are valid, even with my results now I’m still feeling them, but in the case of my last two pregnancies post my first very unlucky one, they have both been absolutely fine and it is so rare to recur

I also wanted to let you know to push your midwives and screening teams at the hospital you’re in to give you the testing as it is definitely possible and make sure you let them know how anxious you are x

Thank you so much for your reply! I just need to hear a story like that to give me a little boost, it’s so hard waiting and trying to stay optimistic.

So happy for you that the unlucky first experience was just a one off, hope I can say the same in time 😊

Appreciate your advice too about advocating and pushing for screening and scans. I just contacted my screening midwife yesterday and I’m going to have an in depth dating scan with a fetal medicine expert because I’ve pushed for it, so glad I did, I’ll be having the standard screening bloods at this time too, but good to know about potentially being classed as high risk from now on due to previous!

I can’t fault the hospital so far, they’ve given me two early scans so far, very reassuring. Roll on 12 weeks and hopefully positive news so I can start to share the lovely news and relax into it a bit more!

I had really really low Papp a in my first, I don't have it in my second ( I'm pregnant now )

I am 29 weeks hun.

I carried my daughter to full term last time and Low Papp A didn't affect us, so I'm terribly sorry for your outcome!

There can be many positive Low Papp A stories too. The Facebook group is really good and informative with lots of women x

Aww that’s wonderful 😊 and thank you for that, I joined the Papp-A FB group last time but I’m trying to avoid getting too obsessed by it before I know anything, I think I’d drive myself insane looking at things everyday. Hopefully I follow suit like you and levels are different this time around 😊 I’ve certainly had a lot of morning sickness which I didn’t have at all last time!

How are you getting on? Did you have any joy with getting a NIPT?

We terminated a baby at 14 weeks for T18 and a cystic hygroma and have just found out we are pregnant again. Feeling a bit daunted about what's ahead of us but also excited and hopeful.

My dating scan will also be done by a fetal medicine sonographer so this will hopefully give me the reassurance.
