NIPT before 12 week scan, still want Papp-A

Hi all. 2nd baby, age 40. I’m 11 weeks and opted for private NIPT at 10 weeks, which just came back all low risk. Didn’t do this with first pregnancy.

My 12 week scan is coming up. I’ve read lots of people say that the NHS won’t do the NT/combined screen if you’ve already had NIPT. But my first pregnancy I had low Papp-A detected at the combined test so I want this performed still. Is this going to be a problem? I wasn’t going to mention the NIPT but I won’t lie if asked. Will they categorically refuse to do the Papp-A bloods even if I want them? Anyone have any experiences with this situation? This is causing me massive anxiety!

I would just not tell them you’ve had NIPT. You had it done privately - it’s no one else’s business.

Alternatively, if you feel for whatever reason that you have to tell them and they then refuse your 12 week screening, be prepared to pay privately to have that done.

I don’t plan to say anything unless directly asked - and I wasn’t asked during my last pregnancy (although I hadn’t had it). And you’re right, worst case I pay for it privately. But hopefully it’s my “right” to have it on the NHS. Just couldn’t believe how many people are refused the bloods when low Papp-A leads to different management - although this depends on the trust as well.

@peanutbutterandfluff not been in this position myself but tbh it sounds like it is your right to have this test - particularly in light of your medical history. I’d imagine you’d have a pretty robust complaint if anyone tried to refuse.

In terms of pregnancy outcomes, low papp-a and placental function can be significant, as well as interlinked after all. I was repeatedly told this when I experienced low papp-a.

I hope you’re able to feel reassured when you have your 12 week appointment. I’ve not personally heard of anyone being denied this test when they’ve asked for it, but have come across lots of people who opt out as they see NIPT as a superior screening, but don’t always understand the significance of the NHS tests aside from trisomy screening x

I had nipt through my hospital and was encouraged to still have the NHS screening for the papp a

You can absolutely get the combined one on the NHS. I had it twice after private NIPTs and this pregnancy I had the NIPT on the NHS due to previous chromosomal abnormalities and my midwife, consultant and Sonographer encouraged me to still do the combined test purely for the Papp-a. It's your right.

Just had the NHS 12 week scan and the combined blood test, no issues getting it. They didn’t ask about Harmony so I didn’t say anything. Didn’t even ask when I’d said I’d had private scans.

Thanks everyone for your info and support! Now just to wait for the results! Scan was all good though so fingers crossed!
