Handing in notice

I work in healthcare and i have been in my job a year I have loved lots of aspects of my work, however the environment has become increasingly toxic and unsustainable. This has been caused by only a small amount of individuals, but had a big impact on me (think workplace bullying, passive aggressive manager, name smearing) In addition I have had a very hard year personally with health scares and a parental death.
I took some time off as a result of this and have found and accepted a new position. I am very much looking forward to the fresh start. I am handing in my notice tomorrow and feeling nervous about it. I am concerned about my boss giving a poor reference and also that my absence could count against me and my new job offer might get recinded.
YABU - it will be fine people move jobs and the reference will be fine.
YANBU - you should be worried about job being rescinded if these things flag up.

5 weeks but I did loose a parent so was circumstantial not something I would do regularly

Was the bereavement absence recorded as sick leave?
A factual reference can include your sickness record, but not every employer includes that and not every new employer expects it.

It was classed as sick. I feel more positive about things today and like things will be ok and I’m just overthinking,

Good. I'm glad you're feeling more optimistic.
I'm a year into a new job, in a totally different field with brilliant colleagues. I've honestly never been happier in a workplace and I hope you feel the same in your new job. 💐

Isn't there a different explanation for bereavement leave?

Having worked in absence management the overall impression of the absence may depend on whether your current or future employers wish to express it as the number of weeks of absence, the number of spells of absence or by a calculation known as the Bradford factor which is twice the number of spells of absence times the total number of days. It therefore weights a lot of short absences more than one or two longer ones e.g. someone who overdoes it at the weekend all the time and has 10 Mondays off would get 10 x 10 x 10 = 1,000 whereas someone who takes six weeks off for surgery would have 1 x 1 x 30 = 30 (assuming they each work five days a week).
