Future planning

I've seen planners that allow you to list your preferences post death. 'oh shit I'm dead, now what' planners, does anyone for a version for getting old/ dementia. I've googled and can't find anything.
There is a strong familial pattern of dementia in my family and I'd like to put what I'd like to happen in a planner that family will understand and care agencies will try to follow should I succumb to make it easier on my DC.

I'm fully aware cognitively and show no signs( in my 50s) I'm thinking how to do things for the future. Already downsized our home. Paperwork all in one place. As we all know it's not an easy conversation to have and it's things like, 'don't feel obliged to look after me, if I'm not capable I will need to be in a home whatever I may say when the time comes' 'I will want 'such and such' memento' Dementia can make people act out of character as we all know. I've no assets/ money so legally easier in that respect in some ways. Although practically it might not be, I don't know. I've seen the oh shit planner, which I'm planning on using, but that is after I'm gone, I wondered if there is an equivalent.

It sounds like you’re wanting to do Advance Care Planning. Look up ACP and you’ll find lots more information. It includes LPA and Advance Decision to Refuse Treatment and also Advance Statement of Wishes, things you don’t want, types of decisions you would make, what’s important to you, what would constitute a “good” quality of life for you etc.

The way I’ve approached this is to have very detailed instructions included on both of my LPAS (worded and tested by my solicitor). Plus I’ve left detailed written instructions which my attorney (life long friend knows me very well) and I have discussed and agreed upon.

also done my own version of ‘after I’m gone’ including all the usual things (finances/funeral plans/will etc etc) plus instructions of who to go if the executor of my estate decides they just cannot deal with any of it. Again I’ve discussed this with both my solicitor and executor.
