What could be causing this? TMI potentially

My close relative has been having quite severe rectal bleeding for a long time. Just over a year probably. Initially she was very reluctant to go to the doctors but after a while it became clear she needed to. She's had two camera procedures - one colonoscopy and another one but I forget the name. Nothing sinister found but she has internal piles and surgery has been mentioned, which she's had a pre assessment for (she is being very vague on details of what surgery - i don't think she fully knows herself).

Today she has been very upset as the bleeding started again and ruined her sofa. It just seems to happen with no cause, it's not something that occurs after going to the toilet (well it does but it happens sporadically when she's doing nothing as well). Is this typical of piles?

My other relative has gone with her to the hospital appointments so far but I'm wondering if I ought to try to go to get a clearer picture of what's actually happening. Not sure if she'd want that though.

It's all very distressing for her and I'm concerned as well. Since the tests have come back clear I keep trying to reassure her but it can't be right to lose so much blood, so often with no obvious trigger?

Haemorrhoids can bleed a lot. It can come and go. It's not nice, but hopefully she'll get something sorted.

Having had them myself I know they can bleed but it's usually after a bowel movement not when you're just sat on the sofa doing nothing. I think that's what's worrying.

Is she on blood thinning medication, such as Warfarin? Sometimes this can dilate blood vessels too much and cause internal bleeding.
I would go with her to any appointments though, as it seems she's not fully understanding what they're saying to her.
