Any pharmacist or medical specialist here?
I took high dose of vitamin D3 which was 200000IU yesterday.
It was prescribed by a doctor for winter months when I was back home.
Now my only concern is that I am a nursing mum of 20 month old. Because it's a really high dose I'm concerned that if its safe for baby or not. And what to do now.
I called nhs 111 the nurse wasn't sure and told me to ask a pharmacist. I tried the nearest pharmacy but its closed.
If anyone can help me find it'll be so helpful. At the moment I'm anxious.

How often are you taking that dose? It's far above the usual maximum dose recommended without medical advice. The usual maximum is 4000IU per day.

Fifiesta · 17/11/2024 11:59

Have you added too many zeros? Are you sure it wasn’t 2000 IU ?
Where did you get the Vit D3? - I don’t think you can buy what you have put as an over the counter purchase.

Ask on Breastfeeding and Medication Facebook group. Run by pharmacists who focus on providing expert guidance for breastfeeding mums

Calliopespa · 17/11/2024 12:00

Until you find out maybe express and dump. Will baby take a bottle?

LadyGabriella · 17/11/2024 12:01

40,000 units can be given I think weekly when a patient is severely deficient.

This suggests baby should be monitored if you're taking huge doses of vitamin d. I'm guessing you should not be taking such a large dose if you are going to be breastfeeding especially if you weren't told to do so by a GP

Using vitamin D during breastfeeding

Vitamin D can be used during breastfeeding; higher doses require infant monitoring. Recommendations apply to full term, healthy infants only.

Just take it today for the first time.

Yes. I'm sure. Willing to post picture if allowed.
I bought it from back home (Pakistan) where it was prescribed by the Dr.
But I'm not sure now if I mentioned about lactation at that time or not.

Is that a typo OP. How many actual tablets have you taken?

User202320 · 17/11/2024 12:15

Took it yesterday for the first time.

I think you need to post a photo of the packet/container showing the contents and dosage per tablet.

doodleschnoodle · 17/11/2024 12:16

Do you mean 200,000 or 20,000?

The latter is sometimes prescribed as a weekly amount. 200,000 is a huge amount in one go and way over even a loading dose for severe deficiency. Is it a tablet? That kind of amount is usually only as an injectable.

20,000iui tablets are available though.

Mipil · 17/11/2024 12:19

If 200000 IU isn’t a typo, I would call 111 or go to a pharmacy to check the poison database but I wouldn’t be panicking.

Your body makes very high levels of D3 in the sun. One study found 15 mins of midday summer sun produced the equivalent of taking a 10-20k IU supplement. Vitamin D3 toxicity usually occurs after a prolonged period of taking very high doses of supplements.

buffyspikefaithangel · 17/11/2024 12:22

Is it 20,000? I was prescribed that as a loading dose/high dose course

Not a mistake unfortunately.
Its just one capsule.

Tesco or boots pharmacies are open on Sundays if you can get to one.

TomatoSandwiches · 17/11/2024 12:37

I get a 7 week prescription a few times a year, 2 liquid capsules at 20,000 IU each to take twice a week because I'm deficient so that is entirely possible, I can't help you with how it affects breastmilk though op.
I would phone 111 and they can direct you to a pharmacy that's open today.

Salome61 · 17/11/2024 12:37

This is from Wikipaedia - "Those who are pregnant or breastfeeding should consult a doctor before taking a vitamin D supplement. The FDA advised manufacturers of liquid vitamin D supplements that droppers accompanying these products should be clearly and accurately marked for 400 international units (1 IU is the biological equivalent of 25 ng cholecalciferol/ergocalciferol). In addition, for products intended for infants, the FDA recommends the dropper hold no more than 400 IU.[166] For infants (birth to 12 months), the tolerable upper limit (maximum amount that can be tolerated without harm) is set at 25 μg/day (1,000 IU). One thousand micrograms per day in infants has produced toxicity within one month.[160] After being commissioned by the Canadian and American governments, the Institute of Medicine (IOM) as of 30 November 2010, has increased the tolerable upper limit (UL) to 2,500 IU per day for ages 1–3 years, 3,000 IU per day for ages 4–8 years and 4,000 IU per day for ages 9–71+ years (including pregnant or lactating women).[159]"

needsomewarmsunshine · 17/11/2024 12:38

If it was one capsule that would be the dose surely, best to find out though.

crivit · 17/11/2024 12:44

I've just opened up an ampoule packet of 100000 to read the leaflet. It's only half your dose but still higher than most and, although it recommends talking to your doctor before taking it, it also says it can be taken during pregnancy and breastfeeding if that helps?

I once had 25000 / 2 weeks. But its just one capsule.

For anyone confused, very high single doses, or slightly lower weekly/monthly doses are often prescribed if you are very deficient to bring you back up to normal levels quickly.

I took 50,000 tablet once a week for 6 weeks as a loading dose prescribed last year when my levels were basically zero. It comes as one single tablet and higher ones are possible. I only take 4000 a day now maintenance.

My late OH was on 40,000 once a month for the last couple of years of his life too.

@User202320 beware it's very likely you'll struggle to sleep tonight. I stayed awake for 24 hours after each dose as it had me utterly wired.

ThatRareUmberJoker · 17/11/2024 13:05

I wouldn't take anymore until you have spoken to a pharmacist. The one you took yesterday should not harm your baby it would be a different story if you took it regularly while breastfeeding baby. Your baby could get a reaction. I would get advice first before taking anymore. I hope that puts your mind at ease for now.
