hospital letter problem? Wtf?

I was in the hospital recently for heart problems. I was at my GP today and saw that on the hospital discharge letter they had written ‘self harm’ as one of the diagnosis’s… that wasn’t ever mentioned or discussed at all in hospital, I was in for something completely different to psychiatric things so I can’t understand where this has come from. I absolutely do NOT self harm.

The gp said they can’t remove that from the letter because it’s from the hospital but that I’m not down as self harm risk on their system thankfully. But now anytime I go to the doctors or hospital appointments they’ll be able to see that letter and I’m not really happy about that because it’s completely untrue and I have no idea why it’s even on there

Does anyone know wtf I can do about this to have it removed?

@Unhappyperson4 are you overweight /smoker/vape/ drink more units of alcohol than the government recommendation? If yes it might be the "reason" for self harm

Contact the consultant’s secretary that dictated the letter - raise the issue and ask for it to be removed

Nah I’m honestly none of those things, I’m a competitive athlete in my 20s x

@Unhappyperson4 then no idea, but you can contact the secretary of the surgeon and ask for explanation and removal . Hope you get it sorted

There should be a "patient advice/liaison" service in the hospital. You can contact them, explain your concerns over this diagnosis first appearing in a discharge letter and request that your hospital record is amended after a clinical review of your notes.

Or had any accident/illness they suspected was self inflicted?

Things like severely restricting your eating, misusing drugs/prescription meds, overdose (even accidental one) can go down as self harm. Or if you'd been advised not to exercise for a time due to injury or illness but went ahead anyway.

I wonder if a doctor years ago wrote 'SA' on your notes as an abbreviation for something else and a different doctor assumed it was self harm so wrote this in full, so its been copied onto the past medical section for years?

If you've ever self harmed, that information is relevant to any doctor treating you for something else, as people who have self harmed in past are statistically more likely to do so again. So they may ask extra questions about your psychological wellbeing when you present with an injury or unexplained symptoms.

My GP had coded an appointment with a serious physical issue that I don't have so I emailed them and rather removed it. Get in touch with patient liaison as it will be a mistake.
