To be shocked at how much post-op pain I'm in?

A week ago, two weeks into a lovely honeymoon that should have lasted 3.5 weeks, I fell and broke my wrist very badly (displaced distal radius Smith fracture). It was in Japan, they x rayed and assessed me, gave me a temporary splint and was told I had to go home asap for surgery. Sorted insurance stuff and they flew us back mid week, have managed pain with OTC meds plus codeine. A & E Thursday and they got me in for my surgery Saturday as an urgent case. I've had a plate and screws inserted and my fracture realigned with a cast, signed off work for a month.

The nurses were kind, but my pain management for the 36 hours I was in hospital was totally inadequate. They gave one ibuprofen with two paracetamol every 6-8 hrs, I know that's first line pain relief and then low dose oromorph every 4 hrs. I've had oromorph before after knee surgery and it was good, but it wasn't helping this time- not sure if dose too low? It wasn't enough, my pain was still so bad so they reluctantly got me a tramadol to take the edge off. When my nerve block wore off yesterday morning, I was crying in pain and despite asking, my tramadol doses only came 7-8 hrs after the last one (they need 2 nurses to sign for it and it was often hard to find another one to do it), meaning I had severe breakthrough pain that was then very tough to get on top of. My husband was horrified when he came in and saw the state of me. I was discharged yesterday afternoon and thought well at least I can now take my pain meds on a proper schedule.

I suffer from adenomyosis and cripplingly painful periods, and I've had several surgeries so I understand some post-op pain is normal, but this is another level of pain and I can barely get it to a manageable level. I've usually coped overnight with a bedtime opiate plus OTC painkillers, but I've woken in severe pain after 5 hours. I'm taking ibuprofen, paracetamol and tramadol at the intervals indicated, although 4 doses in 24 hours seems to go quickly. Can't use ice as it won't penetrate the thick cast.

My knee ligament repair a few years back was bloody agony but this is another level, maybe due to the broken limb as well. Anyone else who has had surgery on a broken bone, how long until the pain reached manageable levels? I'm just sat quietly sobbing right now while DH sleeps peacefully ( he has been brilliant so I don't begrudge him his sleep). Going slightly mad and still heartbroken that our beautiful honeymoon was cut short by all this pain.

Oramorph won't give you diarrhoea

I'm glad you're being seen. Make it clear you cannot cope with the pain- I've found when using that kind of language it gets through. Sounds like you absolutely should be sent home with some oramorph. Tell them you've been on it before and come off it with no problems, that the adenomeoisis and other surgeries aren't as bad as the pain you're in now.

So I've got a bed in the assessment area and have now been seen by one of the ortho junior docs, she agrees this level of pain isn't normal and they're going to investigate and get some more pain relief for me. They've cut the cast off which relieved a bit of pressure, but it doesn't appear it was cutting off circulation or anything like that which is good. So it will need redoing at some point.

They're doing bloods and urine test to check for any infections but I don't have a fever so good sign. I think the consultant will come down in a couple of hours to review what's going on, but they're also going to have someone from the pain team visit me as well. If they can't get my pain controlled they're going to admit me again.

My tum seems to have settled now after one of the lovely nurses brought me tea and toast. So it prob was the effect of morphine on an empty stomach.

This happened to me with foot surgery and what sorted it was volterol.

It sounds like you're in good hands now, I can totally relate to your post having had multiple surgeries with inadequate pain relief. I hope you feel much better soon.

To add insult to injury, since arriving at A & E I've had explosive diarrhoea three times. Feel all hot and shivery. Maybe it was the oromorph given to me on an empty stomach? Still waiting.

Tell them about this - it could be an infection which is causing your pain and now making you ill.

Update sounds like they are looking after you well. I hope tests come back infection free and you can go home with some better pain medication.

I'm glad they have finally seen you OP and are checking things out for you. Hopefully they can provide pain meds for you and that you don't need to stay in

I've had my cast redone and it doesn't seem as tight, so that's a help. Took two of my own tramadol earlier for the pain and just about managing, turfed back to waiting room now which is packed with patients. Been here nearly 7 hours and still yet to have an x ray or be seen by pain team. I'm so tired and anxious about my painkillers wearing off soon and I still haven't been given anything different.

My mum was in horrific pain after her smith fracture. She also ended up with carpal tunnel syndrome (or something similar) and needed further surgery. I'd definitely echo previous posters to get further advice if things don't seem to be going right.

Can your ring the ward you were discharged from? The consultant who did your op should really be prescribing you better pain relief.

Probably not helping in the immediate short term but can can buy naproxan online from a pharmacy. I did when I got my wisdom teeth out because the dentist refused to prescribe me anything and para/ibuprofen wasn't helping at all 😬

Go to reception/ the desk of wherever you are in the hospital and say you need more pain relief. Do it now.

@DrCoconut Hope your mum fully recovered now, sounds awful. I'll keep an eye on the numb fingers and thumb and ask for a sooner appointment at the fracture clinic if needs be.

Ortho doc came back and said pain team don't advise changing anything 🤣🤣... I insisted and said I wasn't leaving without something else so they've reluctantly agreed to prescribe oramorph. Just waiting for that now and then x ray to check, it's just soooo busy now so might be waiting a long time.

Wtf is wrong with them? Are they staffed by sadists? Makes me so cross. It is unacceptable that they do not accept you stating that pain is unmanageable. Grrrr. Hope they give you plenty of oromorph. Dicks. Feel better. Flowers

It's ridiculous you have to fight so hard for pain relief.

Is your tramadol slow release? SR? If not I wonder is it worth trying to get some to keep your pain more covered? Easier said than done I know

X-ray looked ok so they gave me the oramorph script and I'm now home with it, nearly 9 hours later.

Still have the awful diarrhoea, weirdly enough. Might be the lack of sleep contributing but opiates usually have the opposite effect on me.

Really pleased I got the cast changed as it seems to be making a real difference to my pain levels. But where would I be if I couldn't strongly advocate for myself/draft in DH when needed to advocate for me? Really pissed me off tbh, not to mention a waste of time and NHS resources that wouldn't have been needed had I got better pain relief before being discharged.

Thank you to everyone who posted and was so kind - now to get through the next few days as best I can.

@PrimalLass No that thankfully, cleared up by the evening yesterday.

Had an okish night and managed 6 hours between painkiller doses this time, so have got myself on a proper schedule. A little sleep makes things seem better.
