Desperately need some advice: daughter so unwell

I'm really hoping someone can help or at least provide reassurance.

My DD11 is now at week 3 of having severe stomach pains, so much so she cannot walk without help or sit upright. She is eating and drinking relatively OK.

The pain means she has pain relief every 4 hours, paracetamol and ibuprofen on rotation. She is asking for pain relief an hour after the last dose. It does very little to make her comfortable. I have tried to entice her to do things - and even the bribe of visiting her favourite shop and buying a treat (which would involve very little walking, just from the car park) resulted in her crying "I can't, I can't walk with the pain". At night when she eventually manages to sleep she cries or whimpers in her sleep. We're awake continually due to the pain.

She has had 2 ultrasound scans (no appendicitis or anything sinister) - they say her lymph glands in her stomach are inflamed (but only 1). Been told its mesenteric adenitis.

Have had private GP assess her. The bar seems low - if she can eat, drink and poo then she is deemed "clinically well". Saw another GP last night after she suffered a day of diarrhoea (new symptom). I ended up crying - which probably wasn't helpful - after we just went around in circles. Scan is clear, bloods are relatively clear (low markers but expected with a virus), urine test clear. Tilted head from GP yesterday "what would you like me to say or do".

My gorgeous daughter, who is usually so full of energy and laughter and endless bloody questions, is silent, and I can see her getting thinner and thinner, and more miserable. I feel like I'm one of those mothers who is making things up as I just can't seem to get answers (main question - when will she start to just feel better - not even get well, just a tiny bit better?). No one seems able to answer.

What can I do? Is mesenteric adenitis really like this - no improvement at all for 3 weeks?

but please don’t overrule a medical professional that actually saw your daughter and even her scan on the basis of AIBU posters

I’d suggest trying to get her back in for a very short period. Time it so that she’s dropped off just after having some paracetemol and see how she gets on for a couple of hours.

@JANEY205 No - and I mentioned this yesterday at the doctors, and they agreed it would be a good idea. Problem is her bowels are very very empty now after 5 lots of diarrhoea so might have a wait of a day or 2.

Have you weighed her? Documented weight loss is helpful.
Don’t worry about masking symptoms. Lots of things have been excluded,
Has anybody mentioned abdominal migraine?

The treatment for MA is explicitly

Children often have abdominal pain associated with viral upper respiratory tract infections, due to enlarged lymph nodes in the abdomen, this is called mesenteric adenitis. This can be managed at home with Paracetamol and Ibuprofen.

added to which your daughters doctor also prescribed

@khaa2091 Yes, the migraine thing was mentioned.
@Nuttyroche No all medical professionals, that was unfair and coming from me being ratty and stressed and sleep deprived. However - this was all the GP seemed to focus on yesterday, and if she's going to be in pain long term I'd like to know what is going on, what to expect, when to escalate or go back etc.

There is another thread almost identical symptoms I wonder if their is some kind of stomach bug going around. It is the time of year where kids have just gone back to school and catch everything

@Differentstarts Yeah I did see that one - young boy with stomach pains but seems to be a bug. But he's bounced back quite quickly, as DD did after the initial D&V. The lingering pain is now 2.5 weeks old which seems ages now.

My DS had mesenteric adenitus .. it was awful for 3-4 weeks but then just got better . I was so relieved.. he did then have a second bout a few months later but not as bad .. another whole week off school though . Hot water bottle helped a little

I'd be concerned if she's losing weight. Apologies I've not read the full thread and am shattered but medical - have they considered sending her in to local paeds assessment unit? Crohns can present in preteens but it's a relatively short history.
If she's losing weight take her back to the ' normal' GP
Is the Fybogel helping ?? A GP gave it to my son once couldn't for the life of me work out why. Horrific stuff

Has any doctor not recommended buscopan for stomach cramps? I had an e coli infection which cause severe inteational cramping. Only thing that worked. Search on Mumsnet for Mesenteric Adenitis and you will see similar threads with advice and what helped people. Honestly.. not eating much and buscopan.

Agree with this buscopan is a really good drug

@stayclosetoyourself we went to the local paeds unit initially to rule out appendictus. Since then its been chasing for help, and good old Google just says MA is manageable at home with painkillers etc - yet they don't do anything to help at all.

Thank you to those who have experienced this, it really helps to hear your stories even if there is no quick cure.

Is she having soluble/liquid medicine? Or pills? If first that definitely won’t help with sickness/stomach. Sorry no advice. Other than don’t give up and who cares if you look crazy

Also I got him to rate his pain out of 10 and was usually around a 7-8.. I knew things were improving when it came down to a 4.. such a worry. Hope she improves soon x

@VirusSchmirus the lansoprazole will work to protect her stomach, it's what I'm prescribed when I have to take high dose anti-inflammatories.

It does sound shit but echoing your GP what actually do you want? Do you feel she needs to be admitted? Has she had any x rays or scans other than the ultrasound? If she's eating and drinking and emptying bowels and bladder that's less concerning than if she wasn't able to manage any/all of those.

Dc2 had abdominal migraines. They didn't last for weeks on end but often days, feel better for a day and then get another. He was prescribed propranolol and we also got him some therapy (mostly CBT based) for anxiety which was fantastically helpful. He hasn't had one in the last 9 months, whereas before as I say they were pretty regular.

I know you're keen that this is physical but maybe some mental health support for her would be helpful too? She must be feeling like shit mentally too if she's usually active.

Ask GP for faecal calp stool test - symptoms are sounding very IBD related. The pain can be absolutely debilitating

Could she be tested for gallstones? My sister had a lot of stomach pain for many weeks but I think gallstones were only mentioned as a possibility when she became very jaundiced one day. She had surgery to remove them at that point but I think hers was a severe case. Wishing your poor daughter all the best i hope she gets better soon.

Don’t discount IBS. It can start after a virus or other infection. It can be horrendously painful, and the symptoms vary.
There is no diagnostic test for IBS and traditionally it’s diagnosed when other conditions have been ruled out by testing.
Some PPs have suggested an antispasmodic such as mebeverine. This is very helpful for some sufferers.
If this pain persists much longer, consider seeing a registered dietitian who could help identify food triggers.

This may sound a bit left field but have they checked for an ovarian cyst . There was a poster on here a few years ago with a dd about the same age and similar symptoms and it transpired that they can happen pre puberty.

I had this exact same thing when I was 11. They said it had the same diagnosis. Prior to this they had looked in to Appendix, hip problem, kidney infection, periods (I hadn't started). It did get better then came back again.

My mother paid private. Doctor decided to remove appendix when pain returned. Never had the pain after this and was told it could have been a grumbling appendix.

I hope you get to the bottom of it as I remember the pain being horrendous I couldn't walk unaided either x
