Back in hospital for my bleeding bum

It’s me again!

Recap: IBD flare with heavy rectal bleeding for four months, couple of hospital stays, couple of referrals that still haven’t happened, and worsening ever since.

I got readmitted to hospital yesterday as my bleeding has got worse, and I am not sleeping properly due to being up on the toilet til 3am onwards.

I’ve tried to continue work throughout this time and made the decision to come here yesterday when I was bleeding so heavily with clots that even standing up blood was dripping on the floor.

Recently I’ve had a change - I’ve started getting intense lower abdominal pain and an urge to go to the toilet to the point I feel like I’m going to have an accident; it feels like birth where you HAVE to push and your thighs hurt even though it’s just blood coming out.

I have now developed a mass coming out of my anus which looks like my rectum, but it goes in after I’ve had a bowel movement (slowly).

i managed to take a picture of it (with difficulty!) and the red mass is half covered in thick white which looks like an ulcer. Would post a picture but I think it’s way too gross!

The A&E doctor said it could be a haemmorroid (irk how to spell it) bleed (?) and when I showed her the picture she said she can’t confirm by a picture and needed me to see gastro.

I saw a gastro consultant briefly who agreed it looks ulcerated and wanted me to stay in for pain relief (I was literally SOBBING) to see the rest of the team today.

I still haven’t seen anyone from the team but im
super worried that when I do they are going to say it’s hemmorroids when I know it’s not (have had internal exam and camera and you can physically see the ulcers).

im just wondering if anyone can help me on what the main differences are between an internal prolapse and hemmorroids because Google is absolutely no help at all, and I don’t quite fancy posting this as a Facebook status!

no one here with me to advocate for me so I’ve really got to push for help properly myself. Any advice or input is massively appreciated.

Have they given you pain relief?

Sounds like, as you say, bleeding is due to your IBD, however, unfortunately you could have both IBD and haemorrhoids, with haemorrhoids being the cause of heavy bleeding/ partial cause.
You describe either a prolapsing haemorrhoid or mucosa prolapse. Colorectal surgeons could work with gastro to have a look for haemorrhoids, how long since your last colonoscopy/ flexible sigmoidoscopy?

If gastro aren't sure of bleeding cause, a proctoscope examination by a surgeon might be a good idea. Better at looking for haemorrhoids and prolapse than a flexi sig.

Have they not asked oncall colo rectal to see you?
Poor you. I hope you get some comfort from morphine.

That sounds awful. I too have bowel issues but they pale in comparison.

You poor thing, that sounds traumatic.

I have no advice on what they'll diagnose, but if they say something you don't agree with/don't take enough care with you, ask for a second opinion. You must advocate for yourself.

Do you have anyone you know with you?

Sounds like a normal cluster fuck job at the woeful NHS…at least at my local hospital

Im sorry op - I’d hold your hand and advocate for you if I was nearby (fellow IBD sufferer)

Id ask to see the surgeon to rule out piles, and then ask to see the gastro team urgently.

I third (?) the advice to see a colorectal surgeon, if only based on experience with dh that the surgeon took his situation so much more seriously (dh did need surgery - which was a big deal, I won't minimise it, but it removed the IBD completely) It really sounds extremely severe , you poor thing!

Wih dh, The gastro team managed to completely miss a rectal abcess for example, surgeon spotted and dealt with itstraight away. Not suggesting that's what you have, I have no idea, but the teams just happened to be chalk and cheese, gastro vs surgical in our trust at the time (while ago now)

Kick up a stink if they try to discharge you, if you can find the energy

Ask for the ibd / colorectal nurses to come by and see you as they can be pretty effective at advocating for patients

Just wanted to say thinking of you - sounds horrific and if they think its piles (easy way to spell Hemorrhoids) you wouldn't be on Morphine and oramorph so they defo know something else going on here. I hope they sort it and quickly.
