What is the correct way to report an incident my manager is brushing everything off?

Go higher then your manager?
Ward matron? Director of services etc.

Imo, and working for NHS you need to be persistant - the effects of said incident on patient wellbeing, staff morale, building infrastructure, obviously only those that apply to the incident.

Keep reporting until you feel your complaint is being answered to, make sure you document and document again.

Freedom to speak up guardian would be one route.

If it was a clinical safety incident report via your incident reporting system. It will be on your intranet page they may call it Datix, inphase etc. If you're not sure call switchboard and ask for clinical risk team who will advise

If its a HR issue call them

If you're finding no one is listening go to freedom to speak up champion or guardian.

Also you could speak to the clinical governance/patient safety director about issues you have and they will deal with it or signpost you.

Also look up your whistle blowing policy.

Alternatively a union rep
