If you start a new job and get no support.,,, your fault?

I've been in jobs where you get minimal support. Minimal training etc.

Is that my fault for not reaching out or should managers be booking in time etc with staff?

It's a bit of both, I think. There should be some kind of orientation programme set up, but also questions should be asked if training or information is not forthcoming.

It does depend on the level and type of job, and the experience you have when you start. Jobs that are very procedural or systems-based usually need structured training for those new to role. With other jobs you can learn as you go by asking questions as you pick up tasks.
