Really peed off with doctor..

So for 8 weeks or more I've been experiencing a fair amount of blood on the toilet paper when I go for a poop. I have IBS so have loose stools isn't unusual for me.

Last week I had THE MOST EXCRUCIATING PAIN in my bum hole I have ever experienced, far worse than childbirth, that lasted hours.

At my doctors now you have to fill an online form to be triaged and you may be seen in a week, luckily I was seen the same day (last Friday). It was a nurse practitioner. Now, if I had a patient come in with bleeding from the bum hole I would most certainly check to see if there was anything visible, but nope, so gave me suppositories and cream, the suppositories are so fking hard I can barely get them up without crying in pain and the cream has done nothing.

Anyway, yesterday, I went to the loo and it was like a literal blood bath. Apart from the normal eye watering shitting glass agony, feeling like I was going to pass out, the loo was full of blood and each time I have been since, the blood is awful, even to fart I am losing blood.

Am I being unreasonable to think the NP should have at least checked the area? I mean, it's 6am and I'm laying here after going for a wee and blood is dripping from my bum.

I'm waiting for 8am so I can fill out the poxy for again to hopefully see a doctor!

Blimey OP, I hope you get seen today.

Everytime I drive past a pharmacy they're queuing out the door. I don't know what is going to happen.

In the meantime, I would suggest anal fissures which are little tears. The pain is more than you can imagine. Get some lactulose. It softens your poos so it won't hurt as much. Anal fissures also cause lots of bright red blood.

That level of blood loss I wouldn't even be contemplating a Dr about. I'd be straight to A&E.

What you describe sounds like a fissure
VERY painful and take a long time to heal (might need surgery in the end)
I would not be going to A & E

I had this in December. It's frightening. I felt like a tear and the pain was unbelievable. I tried using cream for piles but that was too sore to put up so just used in the outside. Lots of warm baths. It did slowly recover. I didn't go to the doctor but should of.

With the pain it does sound like an anal fissure. Sit in a warm bath for 20 minutes x 2 a day, ideally with coconut oil in it. Avoid haemorrhoid cream as that reduces blood flow to the area but you want the opposite, an increase in blood flow to promote healing.

I'm assuming BTW that the blood is bright rather than dark red, and not actually in your poo. If it's dark red go to A&E.

Blood in stool is different from bleeding, blood in stool could be bowel cancer. Don't understand why they didn't say bleeding though.

They've put no blood in stool, not that there has been no bleeding. @TheSaint01, I have haemorrhoids and get exactly what you've described re the bleeding. But I don't have any other symptoms and it usually stops after a day. Certainly my GP wasn't concerned.

Given you're experiencing pain, bleeding outwith passing motions and the previous treatment appears not to have done anything, I hope you get seen today and insist on a check of the area to make sure it is nothing more than a fissure/split haemorrhoid that will repair in time even if it is uncomfortable.

She’s circled no blood in stool - that’s different to no bleeding. Blood in the stool is not what you describe.

As PP have said, having blood in your stool is different to just having bleeding. So she's not wrong with what she wrote.

Anal fissure. Unbelievably painful. Doc should have had a look so bad on them but they have clearly made a mistake re the no bleeding. Go back to get them to look and confirm, say it had to be an emergency appointment. But don't go to A&E.

As PP said- do not use the suppository. It makes it worse. Also an anal fissure will cause your muscles to seize up so it makes it so hard to get a suppository in.

I had this and was going down the route of surgery as nothing seemed to get rid of it but unbelievably it was daily Intermittent fasting that helped me. Less bulk and allowed time to heal. Apparently you do more healing when you are not eating. I started it for weight loss but discovered the amazing health benefits of it. I read delay don't deny. I didn't it for a few months and lost weight but then realised my 5 year old fissure had healed,

The only relief I ever got was lidocaine but there were supply issues when I was prescribed it so I couldn't ever guarantee to get it from the chemist despite having a prescription

Vaseline on the suppository to help insert it is my top tip. Haemorrhoid flare ups are incredibly painful and I get a lot of blood.

I wouldnt dwell too much on the lack of physical exam, it rarely if ever changes the plan and diagnosis which is made from the history youve given. With time constraints in theory it's more practical to focus on the consultation and omit the physical, screen for red flags (for cancer generally) and encourage first line treatment. Then reconsider if recontact/failure. Sounds like they may have thought haemorrhoids instead of the fissure which what it sounds to me like.

They are often left to hopefully heal naturally, making sure you're not constipated, sometimes a relaxant can be prescribed to reduce symptoms. The duration of your symptoms is getting on a bit though so if it was going to heal itself I'd be expecting some easing of your symptoms now so returning to the surgery sounds quite sensible.

To highlight what I mean by often physical exam doesn't change plan - so say you're examined, if I can see a haemorrhoid this might be something you've had for years, many people do so may or may not be associated. Then anal fissures often cannot be seen due to contraction, so if I looked and did or didn't see a fissure I'd still believe that was the cause because of how you describe your symptoms. So ultimately I would have performed an intimate physical exam, taken the time to consent you for it/get a chaperone (this time will mean less time to explore the history/how it affects you) with very little chance of it changing my diagnosis.

Its certainly not wrong to perform a physical exam, just for many consultations it's not required.
