daughter is in a mental health unit. She’s on 1-1 obs she requested to go to the toilet. The male staff member swapped with the female. When he came back he said to the female member of staff “went” as if he didn’t believe she needed the toilet which I find very odd.

He had most likely said it so notes could be updated around regular bowl and bladder movements

It sounds like he was just reporting that she'd actually been to the toilet.

Also, I'm wondering whether they have to keep an eye on service users going to the toilet in case they're accessing the space to make an attempt on their own lives?

I'd imagine it was just for noting (and also if she's going to the toilet and it's not to actually do the toilet, then they might be worried that she intends to harm herself in some way when she's out of sight). What is it you're concerned about?

Doesn't sound like that's what's happened? He swapped with a female member of staff and then just asked that staff member if she had done anything, is my understanding of the OP. Perhaps there will be some clarity with next update!

Could they have been talking about someone else? Presumably the female staff member was supervising someone else then swapped to dd for the toilet trip.

Totally normal. Everything is to be documented in this situation: toilet use, food intake, fluid intake, mood etc. Playing devils advocate if your daughter hadn’t urinated for a significant period of time, then they would want to investigate and for her to see a doctor

She's being observed continually, it's for their/her notes, totally normally.

This is almost definitely for clinical notes. She's being observed 24/7 - this is an observation.

1-1 obs means just that, a MH nurse will be within arms length every single minute of the day and night. It is as expensive as it gets. You only get 1-1 obs if you are at serious risk of harming yourself or others. Short of breaths in and out, everything will be noted.

Your DD is obviously very ill, why have you chosen toilet trips to be the hill you die on?

If I need to 1 to q somebody it's either because they are a high risk or self harm/ risk to others or pts with dementia, high falls risk/ aggression risk. We do need to ensure they've been to the toilet and not something else.

Totally normal, they need to know if she is urinating and her bowels are moving. Ig she is there for 1:1 obs they document everything.

Are you in frequent contact with your daughter or how did you know this?
