You say to someone in a text that you hope they are well/OK etc but they don't reply to that part or ask the same question back?

Meh. It's like "how are you" - everybody says it but that doesn't mean they're expecting a detailed/ honest response.

“How are you - are you well?” = question, generally polite to answer.

“Hope you’re well” = social nicety, signalling a polite intro, not requiring any response.

If I’m ill and the main reason someone is messaging me is to see how I am, of course I would answer the question, and would ask after them too. But generally, I’d just see it as a greeting; particularly if the rest of the message is about something more specific. For example, if someone messaged me saying ‘Hi, hope you’re well. Do you fancy joining me and Jane for a drink on Thursday?’, I’d probably just respond to the drink question. That’s the real reason for the message.

"Hope you're well" isn't a question imo, it's just an opener.

A question like "How are you?" is polite to respond to.
