To think I might have lipedema? Anyone know anything about it?

I’ve read around 10% women have lipedema, many of whom go undiagnosed.

Ever since puberty, I’ve always had very flabby upper thighs and large buttocks compared with the rest of my body. I’m skinny on the top half of my body, but my bottom half is really wobbly. My muscles aren’t strong but if I bulked them up, the fat would still be there - I know this because I have been a stone underweight in the past when I compulsively exercised and ate barely anything (unfortunately I was anorexic as a teen, but thankfully have been well for around 20 years now). Even then, I could grab a whole handfuls of it, which is part of what kept me in the grip of anorexia until I realised it was too miserable living life like that and started

My thighs currently look very similar to the attached image (insta pic of a young woman who has lipedema). I also have an awful lot of cellulite - the kind that practically throws shadows in places!

Exercise-wise now, I walk a lot, but don’t run (bad knees) or go to the gym. In the week I don’t tend to walk over 8,000 steps a day, but on weekends it can easily be above 20,000 each day. I walk swiftly every day so as to get my heart rate up and always run up the stairs at home.

My ankle on one side has a swelling that my GP previously said was a lipoma, but now I’m wondering if this is part of a bigger issue. I also have optic nerve drusen (fat deposits on the optic nerve) and a cholesterol that is on the higher side of normal, despite a healthy diet.

My BMI is around 19.5 (so I am still on the slim side) and my upper half looks skinny which I don’t like, but I never gain weight there. I also have slight bat wings under my arms.

I had liposuction around age 20 to remove my saddlebags on my outer thighs (plus a revision surgery). They are still there a bit, but considerably less noticeable than they were. They were very prominent prior to surgery and looked rather odd compared with my bony top half - they looked very like this: www.instagram.com/p/CEHrDM2gt1N/

As I get older, I have stopped wearing normal socks as I get a huge sock line at the ankle that lasts for hours. I wear trainer socks now. My legs get slightly bigger by the end of the day, but probably only I would notice. When it’s really hot or I have eaten something really salty, they swell as well (especially with the latter).

My query is, could it be lipedema if I have no pain? The ‘lipoma’ on my leg aches a bit sometimes, but nothing major. The rest of the flab is not painful.

Or am I just ‘skinny fat’? I am conscious that if it is lipedema there are things I need to be doing to prevent its progression. Problem is, I have a feeling it would be very hard to get a diagnosis because right now I don’t look particularly out of proportion - a passer-by might only look twice if they saw me in a swimming costume (which will be NEVER, haha) and I have read that most GPs don’t know much about to the condition.

It's awful that treatment for this hasn't been available on the NHS for severe cases.

I've always had bigger thighs than the rest of me and cellulite unless I'm super skinny but there is zero fat below my knees.

My legs can really hurt if DH gives them a playful squeeze, really not hard, but I dont have any nodules etc. Think it's just where I carry my fat.

I’m a doctor and I do know about lipoedema. There is some debate about whether it should be regarded as part of the spectrum of normal fat distribution or a separate entity . As with any condition such as breast asymmetry the NHS decides whether or not treatment should be paid for by the NHS depending on the impact on quality of life.

To me having been part of this process with patients the answer is very definitely no in your case - this is a cosmetic issue and I think you should learn to live with it ( in the nicest possible way).
I wonder given you say there are very obvious sock marks whether you have an element of lymphoedema is could this be partly an issue with fluid drainage from the legs. If you push your thumb into your shin does a dimple appear and stay for 30 seconds? If so then compression hosiery would help.

this is a cosmetic issue
I think this is really shortsighted. It can affect mobility and veins. I have had varicose veins removed twice because of pain. My surgeon said that losing weight off my legs would help but I can't do that. I also have knee problems. My BMI is fine and I am a runner and swimmer. I actually don't really care that much what my legs look like I just want them to work.

I agree...

mines is manageable but only because Im much older.. my job keeps me fitter.. and I work hard at fighting it the best I can.. but its still evident... this is no way possible for everyone.. lots suffer so badly.. my cousin can barely walk.. she needs help and soon... I hope the NHS helps her 🌸

I know this is an old thread but I think I have lipedema too. I have a scheduled call from my GP about something else tomorrow and will ask about it. Not holding much hope. Any advice about how to be taken seriously? Do I ask for a dermatologist referral?

Most people I know have paid privately for a diagnosis. I went to see three specialists because I got a 'No you don't have it', and two 'Yes you do'.

Anne Dancey is a consultant who is able to diagnose and who diagnosed me. It costs around £200. She is not working at the same hospital that I saw her at I don't think, so googling her will help you find her.

This is great, thank you! I didn't know about compression or MLD. I only found out what lipidema was earlier today googling exercises for toning my huge thighs and fell down this rabbit hole. It makes perfect sense and I have a lot to look at.

Treatment includes changes to the composition of your diet, use of compression stockings, bandaging affected areas, low-impact exercise, massage and water-based activities. Specialised liposuction for lipoedema can assist when other treatments have not been effective.

I've just been diagnosed with lipoedema by a therapist who specialises in deep tissue massage and manual lymphatic drainage. She said she had no therapy to offer me that would be useful. I've just texted her to ask if she would do a referral letter for me. I can't get an appointment with my GP but who do I go and see for advice. The therapist thinks I'm stage 1, but I don't want to get to stage 2, it looks gross.

What the OP picture looks like to me is that they have fat deposition in areas with fat cells that are likely very high in A2-Adrenoreceptors.

Fat cells rich in A2 receptors are very sensitive to insulin levels.

Even small increases in insulin levels will cause the fat cells there to not empty.

So I've taken the plunge and have booked in with Dr Wolf in London for liposuction on my upper arms as a cheap(er) experiment to see whether I should save up for my legs. I'll report back. In the meantime, can anyone recommend someone for MLD massage (I have to have around six sessions post-surgery)? Central or SE London would be best.

Well, hold that thought because all the surgeons are panicking over the latest NICE guidance, and some are now suspending surgery until they know they're covered by insurance Hmm when did you book? x

????? I read the NICE guidance and it said liposuction for lipodema can be offered on the NHS as part of a research trial. That was disappointing but how would that affect private surgery? Do you mean medical malpractice insurance? Even my arms were just 'normal' fat as opposed to lipedemic fat the treatment is the same.

I'm booked in for the end of the month. I was sent a pre-op consent form today.

Because the professional association for cosmetic and plastic surgeons wrote to its members yesterday and told them to suspend surgery on women with lipodema until further notice. If you were in any of the Facebook groups you'd know this. Dr kolar in Harrogate has actually been quite difficult about refunds too. And no, I mean the surgeon's insurance, they're unwilling to operate as they're not sure that they are insured to do so. Yes it is ridiculous Hmm I'm booked in or 2 rounds of lipo with Dr Gupta in a few weeks and am waiting to see if it can still go ahead.....

I have lipedema- diagnosed.

A ketogenic diet and regular exercise and strength training makes my legs look 100x times better.

My diet and routine has changed since I became pregnant and I have the fatty lump pockets above my knees, and the all over dimples again.

With careful diet and exercise- you’d have just thought I was a bit chunky of leg.

It might not be the case for everyone, but lifestyle changes have definitely helped me.

Interesting. Could this be a reason as to how I have controlled my symptoms with a keto diet?

I went to Germany (twice) for my Op’s.
7K (all in, surgery, flights, taxi, hotel) each.
It’s a bit grim on the way home but it’s a short flight. Plus the surgeons there are the most experienced and knowledgeable.

A lot of people travel for this surgery so it’s quite a well worn path.

I went to Dr Czarnecker, Derma Lymph Berlin. I think though she is only doing surgery for international clients in Poland.

Germany was amazing and so I have two other recommendations:
Although my other recommendations are Dr Klasmeyer in Cologne and Dr Scrieber at Med Plast, Berlin
