What kind of doctor do I need for a frozen shoulder or rotator cuff injury?

I am on my second frozen shoulder - I think, if it's not something else like damaged rotator cuffs. My first isn't as painful now, but has lost a fair amount of mobility. The second is still at the stage where I get jolts of sickening pain if it gets moved a fraction too far the wrong direction.

Would a physiotherapist be a good first port of call? Who is the right person to see to find out exactly what has caused my buggered shoulders?

Not in UK, so won't be seeing a primary care practitioner first.

1 session with sports therapist

Time (18 months), Naproxen, and a v-shaped pillow. Swimming & Pilates amazing for helping. Painful initially, but helped immeasurably to restore movement.

An osteopath

You need an HCPC registered physiotherapist who can also giver cortisol in sections AND if need be cortisol and saline injections (a slightly different procedure). Not all physios are trained registered to do this.
Oh and sympathies

I saw an osteopath, and the results were really good, but I expect that what they did was quite similar to what a physio would do.

I got cocodamol which I took at bedtime only, because getting a good night's sleep really helped. I needed it longer than the recommended 3(?) days, but was only taking it last thing.

It was really painful for a surprisingly long time, so you have my sympathy, OP.
