To expect better care from care home?

My gran is 99 and is in a care home.
Obviously with covid I haven't seen her since March.
Since Wednesday she has slept constantly and refusing to eat.
The home said a doctor did a FaceTime call with her but she was confused (obviously she has dementia)
No examination took place.
They prescribed amoxicillin to treat a infection (as doctor thinks it must an infection of some sort)
I told the home yesterday morning she suffers from uti every 5 /6 weeks and trimethoprim is always given.
Today rang the home,they haven't got her to do a urine sample and they haven't informed the doctor about her recurrent infections.
She is no better
The care home said it will have to wait till Monday now when GP opens.
I don't think that's good enough
Shouldn't they be ringing a out of hours doctor ?
Getting a sample done ASAP ?

Wouldn't the GP know from her surgery records that your gran suffers from recurrent UTIs and takes Trimethoprim for it..is it possible they think it's something else? Sounds like the care home have been pretty quick about getting her 'seen' by a GP if it's two days of symptoms so far.
But, yes, a urine sample for dipping would be ideal but hard to get if she toilets independently.

And, no, OOH not required if she is already on abx and been recently 'seen' by a GP but as said the care home should dip her urine and monitor her obs.

Your Gran is already on abx so OOH is not needed, care home has followed procedure by contacting GP and are following the advice of the GP.
The GP will also surely have your Grans medical notes so will know about the recurring infections already.
You don't mention if your Gran toilets independently or not, but trying to get a urine sample for dipping isn't always as straight forward as it may seem depending on the circumstances.
I am sure if there is no change or a worsening of her condition then the care home will contact OOH when needed and will be monitoring her obs in the meantime.

Amoxycillin can be used to treat UTIs, and since UTIs in the elderly are a common cause of the symptoms you describe, it seems perfectly reasonable for a GP to prescribe that - especially since the trimethoprim isn't doing the trick since the infections keep recurring

No she needs assistance when she goes to the toilet.

They said it was a general antibiotic but when I've just googled it said it can be used as UTI treatment.

I hate to suggest but has your nan an end of life care plan, like if she's not eating or drinking and sleeping constantly and not waking... I could be way off the mark but it sounds like the last days of my late aunt...

Up until two days ago she was fine
Eating,drinking,singing etc
It's just came on all of a sudden.
I think it's some sort of infection.
I really hope it is and the antibiotics help.
