I was started on Sertraline a couple of weeks ago. This evening, I noticed that it feels like my heart is racing (yet when I put hand on chest it feels steady). I also constantly feel like I'm trembling when I'm not.
I can't find anything to indicate either as side effects of the sertraline.
Am I right to think it can wait until morning and phone GP surgery then?

I'm not sure but I'm on sertraline and if j miss a dose my heart rate goes mad

I would ring in the morning, yes. If it feels steady when you put your hand to your chest, I think it's not your heart as such. Plus A&E will be really stressful at this time of night.

A serious side effect can be Serotonin syndrome which can be life threatening - includes fast heart rate. However, I don’t know if this would suddenly come on after a few weeks and what it means if it feels steady when you put your hand on your chest - I’m no medical expert. I believe muscle tremors is also a symptom of this - but I don’t know if this is what you have.

Does the fast heart rate feel “real” or related to anxiety, do you think?

Personally if it feels really off I would call 111, though it is likely they might send you in just with the description, I don’t know.

I would thoroughly check the other symptoms of serotonin syndrome and see if anything else fits.

Called 111. They gave me 2 options, a call back from Go To Doc for further assessment or get someone to drive me to A&E in next hour. As the A&E option would mean having to involve an abusive family member (because they're the closest people I know), I chose the callback.

Doctor phoned. Glad I chose that option. They think it's likely side effects of the sertraline. I'm to phone my GP tomorrow and see if they want me to tough it out or look at changing the medication to something else.

Activation Syndrome and Serotonin Syndrome both have similar side effects, including what you’ve described. Your symptoms seem mild but Serotonin syndrome can be dangerous if it gets severe (usually involves a very high fever). I had similar reaction but much more severe on several medications, retrospectively suspected serotonin syndrome and it always kicked in almost immediately upon beginning a medication for me. Activation syndrome typically takes a little longer to kick in from what I understand and symptoms will pass eventually.

Glad you called them @Jasfc. Sertraline is tough when it's first getting into your system, but I swear by it now. I get harsh side effects when I up my dose or accidentally miss one. Hope you feel better today.

To update, spoke with GP as requested earlier. They were impressed that I actually thought to check my heart rate first. The consensus is that, while it could just be a side effect, they want to make sure. So have an appointment on Saturday morning to get my heart rate checked and need to book and ECG/heart trace.
There was the option for me to just stop the sertraline, but I said that I'd continue it until my review next week and decide where we go from there.
