AIBU to think I might have a ruptured ovary?

History of fibroids and 4 ELCSs - 44 and have had 1 3 month course (2005) and 1 6 month course (2019) of zoladex to treat the fibroids. Had the snip with the last birth. Periods have been one painful and heavy, one light and not painful for a while but getting more erratic. Left pain in hip coming and going for about 16 years. This last 'month' pain started before Xmas with what I thought was auto. Saw a paramedic, leukocytes were up so she prescribed 3 days of antibiotics. Felt a little better afterwards but not much so went back, dip test showed no infection so he did a vaginal swab. Got worse so phone call with GP where he said to drop in another wee test and do some bloods to see if it was my connective tissue disease flaring up. Got worse so went to see paramedic again, where he said the vaginal swb had been discarded for some reason, but he didn't think the left hip pain (now affecting my left flank and to the right of my tummy button) was an infection but he was at a loss so sent me home with some cocodamol. The next morning I had three mini-explosions in my left hip, and 3 hrs later my period started (2 days early, and if anything it's usually late these days). Went to gp to say enough is enough I want a full hysterectomy & oophorectomy. He said fine, it makes sense, I'll refer you, but here's some tranexamic acid to stop the bleeding and some naproxen. This was last Wednesday, and my period's been and gone, and I still have enough left hip pain that I'm in bed with 1 cocodamol and one naproxen and I can still feel the bugger. Wtf do I do now? My life has stopped and I have too much shit to do but I can't see what else to do. Do I go back to the gp again?
I've thought it might be a ruptured cyst but I'm not a bloody doctor and in the many scans I've had, they've never found a cyst before, but this is bloody awful. AIBU to want to go back to the GP? Or should I just wait it out?

If you honestly think something have ruptured, then go to A&E.

But I’m sure 1 of each painkiller isn’t enough...why aren’t you taking two?

Because 2 of each and I'm too zonked to deal with the children and get sleepy and puky!

Ah fair enough

I have actually had a cyst on my ovary that ruptured whilst I was pregnant.

Luckily baby was ok. I had a bit of pain where it was but nothing long lasting.

So yes I would be seeking medical help

And the thing is, is that I'm clearly not dying, it's just fucking painful. I do get dizzy, even sitting down, but that could be because of the meds or because I also have a small brain aneurysm (which sounds scary but it's not that serious) and I have high blood pressure so I'm on meds for that.

What kind of scans have you had, because if you have a thorough internal ultrasound, they can see every cyst and what stage it is - mine took 45 mins yesterday, but 15-20 is more usual.

It doesn’t make sense that you have a cyst pop because of ovulation, because that’s between periods.

Have they ever checked for endometriosis? That would make far more sense. If you’re a fibroids sufferer, they might not have bothered to consider women have more than one issue at a time! Can't see it on scans.

No. It was more like a stabbing pain right above the ovary

Ps I think it would be a huge surgery to undertake without at least the suggestion or an exploratory lap, leading to hysterectomy if necessary.

If he gets in there and finds endo, scarred ovary, something, it might be enough to just take that out. Ŵhat you’re suggesting sounds like a very localised issue.

I would go back to the drs , if this has been going on for a week , three days of antibiotics and a dip test are not always reliable and it could be something else entirely. Make sure you go with someone who will be assertive and don't be fobbed off, just in case its sepsis My mother in law had this even with a clear urine dip .

Definitely seek medical help because you have got several medically complicating conditions and the pain relief you're taking is inadequate.

You need to phone GP and ask for a home visit ASAP as you're in no condition to either drive or use public transport.

At the very least, dial NHS 24 ( I'll be surprised if they don't assess your symptoms as requiring an ambulance).

Thanks everyone. I think I'm just going to try taking one cocodamol and two naproxen and see how that goes for this evening and then go to the gp in the morning. I think I've just reached a state where I'm back and forth to the GP so much that I feel like I'm bothering them and/or a bit of a hypochondriac, so it's nice to know I'm not overestimating things. I do feel like ripping off my left leg now, so I guess it must be pretty bad, even if I'm not crying or screaming in agony!

Have you had a scan since this latest pain got worse? It might be an ovarian torsion, where it twists around, rather than a cyst. Might be a fibroid (they grow and shrink with hormones) or even referred pain from a pinching in the spine or something abdominal. Within a scan, its only a guess though. I'd definitely see a different GP at your surgery.

Thanks elouera - I haven't had a scan since late this last bout kicked off, so i'll see if they can refer me for a scan again - they couldn't see anything last time, but apparently the left ovary was obstructed by the bowel at the time, so that may be why they couldn't see anything.
