Pregnancies are dated from the first day of the last period for the sake of consistency. That is because it's impossible to date it from the day the egg fell out the tube and got fertilised with the sperm. All women know when they bleed so it's an easy, consistent place to "start" counting.
All women drop the egg (ovulate) on different days. Some people will drop it two weeks after the first day of their period. Others will take two and a half weeks. Hence the discrepancy. I dropped mine a week and a half later than the average. My cycle (period to period) must be longer than most.
Add to the fact that babies grow at different rates and on a 12w scan measurements can be out by several days anyway, I really wouldn't worry too much.
So, to conclude:
12 weeks is how long I've been pregnant
15.5 weeks is the gestational age given by medical professionals used for official dating purposes on paper because periods are easy to spot and identify. So they're a good starting point to count from. It's the same for all women.
So this means on paper, I was technically three weeks pregnant when the egg met the sperm!
Can't see why that's so difficult to understand?
