Can hand foot mouth start in the nappy area?
My poorly baby 18 months spent last weekend in hospital with bronchiolitis. Has picked up and almost back to normal. On long term antibiotics which give on/off nappy rash.
Yesterday seemed to get some spots under nappy which overnight have become circle blisters that are broken and weeping. About the size of a babies fingernail each. About 10 in total in nappy area.
Just now seen 3 on mouth but much smaller. Nothing on hands/feet.
Seems well in self - no temp.
Cannot comment on food etc as tube fed baby, no vomit has kept feed down.
Not sure if i need to contact out of hours...(8 hour wait for a call back...or wait to see our normal GP tomorow)
Baby is well and happy, apart from at nappy change 😕

I don’t know the answer but that is how my DD’s chicken pox started.

KTD27 · 14/01/2018 12:21

When my little one got HFAM the first time he got spots in the nappy area. Don’t remember them being open and weeping but it was a while ago. It was the high temperature a few days before and then a mouthful of ulcers which clued is up and the doc agreed was HFAM. None on the hands that time.
Second time hands feet ulcers no nappy area. So it’s variable.
If it is HFAM there’s not much they can do but I’d perhaps get it checked out if you aren’t sure x

Lostmyemailaddress · 14/01/2018 12:22

Yes hand foot and mouth can be in the nappy area
And it can spread down to the thighs. With dd1 it started as a blister in her mouth then blisters on her groin area then spread to her hands and feet. I'm not sure what the advice is now as dd1 is now 9 and she was 4 when she had it. Advice then was paracetamol but id ring ooh to be on safe side.

QueenNefertitty · 14/01/2018 12:24

With hfam my DS was covered head to foot, including down his throat, poor thing

They didn't weep though....

Might be worth checking it out with OOH dr?

bostonkremekrazy · 14/01/2018 12:33

Has not had chicken pox and that was my second thought.
The spots are very different to any kind of nappy rash...

could be thrush caused by the anti biotics. do the blisters look shiny?

bostonkremekrazy · 14/01/2018 12:52

No they were flat spots..no fluid inside, then overnight became open and sore.
Face ones not shiny either..

I would see a doctor and get something to help your baby with this. It can be so painful for them, and you say baby is on (or has been on) antibiotics for something else. It could be related to the anti-bs and the blisters might feel to your baby like a burn in that area. How is you little one coping with that? My one had something similar and was in so much pain that even water (when wiping) and her toileting in a nappy made it worse.

bigfishlittlefishtupperwarebox · 14/01/2018 13:01

When our DD has HFAM at about 16mo it was all in the nappy area and thighs, a few on her chin. It looked absolutely horrible, and sounds very similar to what you have there. She was thoroughly miserable, but it cleared up within a few days, used loads and loads of that yellow cream (metanuim??) and alternated that and sudocream on nappy changes. Hope it clears up soon, there was nothing the docs could give her.

bostonkremekrazy · 14/01/2018 13:09

Baby has been on anti-b's for a long time so it gives a sore bum sometimes...
Apart from wiping which is sore, babe seems completely well...Hmm has had no pretemp which goes with hand foot mouth usually....

Perhaps fungal nappy rash? Google image and see if it's similar. My DD used to get it and it looks quite alarming. Good luck OP

EveryoneTalkAboutPopMusic · 14/01/2018 13:40

Definitely see a Dr today OP Smile

Zelbie · 14/01/2018 14:01

Always, always, get a rash checked out after a baby / child has been unwell.

Please see out of hours.

bostonkremekrazy · 14/01/2018 16:12

Baby has no sepsis 6 or meningitis signs.
Not unwell. Infact bonnier than has been for weeks.
No need for a doctor apart from my need to know what the spots are - do i need to isolate from other people etc....
Thanks all...

Without being funny, have you taken their heart rate, done a blood test, or checked their blood pressure?

bostonkremekrazy · 14/01/2018 16:32

No zelbie, but i'm a parent not a doctor.
Baby has a wierd nappy rash....no other signs.
No decreased tone.
No change in heart rate.
No stopping of breathing.
No jaundice.
Is peeing normally.
Fontanelle is normal.
Rash - only to nappy area, 3 spots to mouth.

So no signs of sepsis according to the nhs checklist online.

No wonder the nhs is on its knees.....

I was wondering if my babe had hfm 😕

OP posts:
Zelbie · 14/01/2018 16:38

Ah, yes, I'm the reason the NHS is on it's knees.

That would be the day i didn't get a weird rash checked out after having a virus for weeks, because I thought it was "just a weird rash / excema"... Turns out it was the start of necrotising factitious. I went from well with weird rash, to ill with weird rash in about five minutes, and thankfully was very close to a hospital. It still cost the NHS a hell of a lot more money, than it would have done had I gone and got the weird rash looked at in the first place.

So apologies for answering your request for advice with, well an answer, but there are very, very few doctors who will tell not advise you to get a baby who's been in hospital, with an illness, and then develops a rash, checked out.

lookingforthecorkscrew · 14/01/2018 16:40

My son’s HFAM started in his nappy area, it looked like bubble wrap. Metanium helped the blisters to dry out quickly. Unfortunately by then it had spread to his face, we had about four days where he looked positively toxic. I kept him comfortable with Calpol and didn’t force food on him, I let him drink as much milk as he wanted.

Scoobygang7 · 14/01/2018 16:44

Could be hfm. Could be some thrush too due to antibiotics. Hope he feels better soon.

bostonkremekrazy · 14/01/2018 16:47

Sorry Zelbie, I didnt mean you particulary.

Just calling OOH GP for nappy rash.....the wait is 8 hours for a GP to call back. When babe was obviously ill last week of course i went straight in to hospital...no probs....

I've spent this weekend in resus with my mum...there is no bed in the hospital to move her to 😕

If its HFM i need to be extra careful as mum is so poorly blah blah blah....

Baby doesnt need a doctor...just a mum who knows whats wrong with her....gah

Hi
That's exactly how my DC2's HFM started. First his testicles swelled up quite alarmingly. Then the spots arrived, all over his bum and bits. Then spread up to his mouth and lower face, his hands, his feet, the backs of his legs. It was awful for a few days, he looked like a burn victim.
We got some amazing wash from the chemist that helped calm it down and gave piriton to help the itching and soreness.
It was a miserable week.
Fingers crossed your LO recovers quick.
X

bostonkremekrazy · 15/01/2018 11:29

Thanks for all your help.
Gp confirmed hfm this am.
Did not do any other obs...just looked under nappy, at mouth, feet, hands.
Gave antibiotics as nappy area is so so sore.
Baby is still bonny and not unwell at all. Bizarre!
