To correct my hospital notes and post them back?

My family think I'm BU and that it's not the important but it matters to me.

I've been seriously ill in hospital and I have quite complex, multiple health problems, so - within reason - I expect some mistakes on my discharge summary. However, stating that I'm a smoker when I've been discharged to a lung clinic when I've never smoked? Getting some important dates wrong? Stating a diagnosis from 5 years ago that ended up being rescinded at it's incorrect?

I don't want an apology, I totally get that mistakes and errors happen. People are only human. However, I do want it corrected on my notes especially as I'm receiving ongoing treatment there. AIBU?

Not unreasonable at all. That's quite gobsmackingly bad. I'm sort of surprised that you don't want an apology.

Definitely get them to correct the notes. You need an accurate record on your files so any future treatment can be accurate and swift!

Contact PALS and get them to have your notes corrected.

Don't just correct it and send them back. That doesn't mean your corrections will be acknowledged or accepted. Raise it as a complaint with PALS and list all errors, and ask for an explanation of how they happened. It is actually important that your medical history is accurate, as any medics treating you in the future may rely on that and make decisions accordingly.

Being called a smoker is very bad - if it sticks on your record because of the mistake it may effect your place on a transplant list of god forbid you ever need one!

Can I complain without complaining though? I had the best care in hospital so I was quite stunned at the discharge summary.

Yes you can. What buttery said. Your treatment needs to be based on correct information. Make sure your electronic patient record is updated. Get this in writing.

COntact PALS (Patient Liaison) as person said upthread. In our local hosp they were incredibly helpful and hopefully will totally understand where you are coming from. Also they will know how the system works and be able to get it done for you.

My doctor told me that a previous doctor at the same practice had noted an operation I had never had! Took me a while to convince them I hadn't.

Just writing a quick email stating that I wanted to formally acknowledge and recognise my great treatment in hospital but I was disappointed at the factual errors then went on to list them and ask them to be changed and a corrected copy back to me and GP. Sound ok?
