AIBU to think that ALL correspondence that is in a patients medical notes should be clearly signed ?
Surely EVERYTHING has to have a named 'author'?

I'm asking because I came across a letter in dcs notes and it's undated and unsigned and nobody will tell me who wrote it. Without a name I can't complain so I'm going round in circles

Surely there's some kind of guidelines (I've tried googling but to no avail) that says letters/emails HAVE to be properly recorded as otherwise anyone could send something in ??

Concerns re: FII are routinely withheld from parents/carers because raising those concerns can put the child at additional risk. Not saying that this is the case with you OP but it would explain why people were surprised that you'd seem the email.

I'd be amazed if the GP records are the only ones to contain this. GPs are usually only copied into such correspondence (and only if we're lucky - vast reams of safeguarding stuff gets sent and we're none the wiser - really helps us to safeguard the children in our care Hmm). I suspect social services have probably had wind of it as well, whatever they say to the contrary.

So difficult for you especially with your DLA claim being affected. If you have an onside GP and they can agree with you that you have diagnostics etc in your favour can you get them to write as much to support your renewal? Or would a paediatrician do so (assuming you have one if dcs have complex care needs). Whoever wrote the original tripe it must be possible to now get someone to refute it if everyone now knows to the contrary?

Seems to be that despite numerous genuine letters from consultants and tests results that this letter is the only thing being closely looked at
This is as well as the fact I had disproved all the other allegations in the letter things like I supposedly had 'daily childcare' and 'household management' paid for I could prove i never had daily childcare and that the household management was a cleaner for two hours a week paid for from our wages.
Things like allegations I had not given accurate info to pre school about medical conditions I was also able to show emails I had sent explaining the exact conditions so I have addressed everything yet still the letter won't be retracted and I can't even find out who did it

Surely if it was all a secret due to an investigation they've realised it's not the case so why can't someone just say to me "x person thought it was this we investigated and it was all fine so here's the letter for dla explaining to them" but nobody wants to resolve this it seems

OP as a GP I would be happy to write that letter I think, going by what you've said on here and assuming there are no ongoing concerns re: FII. Is it possible for you to have that conversation with a GP, rather than going through the complaints process? Sort of "we seem to be getting nowhere re: this 'anonymous' email so can you instead please help me to sort the consequences?"

Yes I think I will ask directly as then perhaps i can start to sort this mess out. I can't bear it that there's been such hatefulness written about me and I need it retracted by another professional.i will ask them to write to that effect

Do you have legal representation? If the private nursing service are refusing to engage with this then it would not be unreasonable to contact a solicitor and query if this is in fact libellous? It is a written and published defamation, and you can prove this. By refusing to remove these notes from a medical file your G.P/NHS is implicated in this. Perhaps a strongly worded letter from a solicitor and a CC'd letter to the Care Quality Commission would result in some names being given.

We contacted a solicitor who said it came under defamation of character. The nursing service is an NHS one (school nursing) so not sure how it would work we are planning on seeing what the the letter says from gp when we get it and possibly going down the legal route after that. The nurse and the previous pre school are both implicated as both have falseness information and malicious lies. Luckily I can disprove them

I would absolutely pursue a legal route. It is disgusting that anyone can imply or accuse someone of anything and it remains part of their written history regardless of whether it is proven or disproved. Good luck and well done for persisting with it. But look after yourself too - this must have been so damaging to your mental health and anxiety levels, do what you can to counteract that.
