To be confused about kids doctor registration form?

We have just moved so changing doctors surgery and without sounding completely thick, although I know I do! but on the form it reads : if you are registering a child under 5 -
" I wish for the child to be registered for child health surveillance " with a tick box.
What is this? The Heath visiting service? Or something I'm completely unaware of even though my kids are now 3 and 4!

I didn't know they were optional!
So you tick yes to have them assigned to you but you don't have to? They cover up until 5 though so guess we will tick yes! Thanks...duh :)

Know! I thought maybe it was only for children at risk or something that's why I didn't know what to tick or not tick!

Child Health Surveillance? What will they be doing? Following you at a safe distance and radioing each other as in The Bill? Sneaking up when you've your back turned, giving your dc's a quick health check by looking in eyes/ ears with their instruments and slinking off when you look round?
What an odd name for them!

I ticked the box in September for my now 3 year old, was two then. All I've had is a letter asking if I want dd to have the flu drops and it's so I get the letter about her jabs too. Must find her red book out now I'm thinking about it.

It's just for vaccinations, HV and routine stuff.

It is a rather unfortunate phrase, but harmless enough.

Even if you tick it, you can still opt out of HV visits etc. if you change your mind.
