when I search for it and scroll down the images I just see fairly bog standard medical type pictures, nothing particularly disgusting or shocking

You will now be on a page with "global preferences" on the top. Scroll down to "safe search filtering", and click on the dot next to "do not filter my search results".

Now scroll to the bottom of the page, and click on "save preferences", a pop-up will appear just click OK.
