Making laminated sheet for teacher

I posted earlier in the year about communication diffiulties at school when there are substitute teachers etc. I spoke to the school nurse and they want me to produce a laminated sheet for the classroom and other teachers, which will be used as a prototype for all children with serious allergies. We are not in the UK and there aren't any clear guidelines in place as yet.

Any ideas what I should put on the sheet and how to word it?
The idea is to have a photo and a very brief explanation of what the problem is (DS is anaphylatic to dairy), what the symptoms are, what he should not be given and the fact that his medicines are with the school nurse.

I'll mention the idea about the colour coding (though at this stage I think it may only be for the children with epipens), and take your point about the name being further down.

It's absolute school policy that all medications have to be with the nurses, and I think only the nurses can administer them.

It's all a bit of a minefield - it seems to be hard enough to find a way to make sure that the substitute teachers will even see the document, as apparently they don't necessarily look through the teacher's roll book, and they seem to think that having the sheet on the door or the wall might stigmatise the child...

I would love to see an example if anyone has one.

Hi, having the laminated sheet in the staffroom might help and other children will not see it, but substitute teachers and other staff will.

Or ask the office to let all substitiute teachers know this key information when they come into school?
