Can I challenge an inaccurate occupational therapist report that has already been sent to my housing officer?

I'm disabled, Five years ago an occupational therapist agreed I needed a disabled wet room in my flat because I literally could not get in or out of the bath etc, It was agreed and installed. I also had a stair lift installed.

Over the last five years my disability has gone a lot worse, my doctor thinks I should be in a ground floor property so I contacted the OT to ask for a report for moving, they came out, did the report and then without me hearing anything else about it sent it to my housing association, I only knew this when I logged into the HA app and found the report just now.

They put my illness is likely to improve. In reality it's a life long condition that will never go away or get better.

They also answered three questions without even asking me about them, And all are wrong.

Say for example a person was in a wheelchair and had no legs, the report would say something along the lines of 'They have no issues walking across the room without any aids and there are no trip hazards in the home'

Long story short.. They wrote answers for three different sections saying I was able to do those things and in reality I absolutely cannot do them. They didn't actually ask me any of those three questions and just filled it in themselves as if they asked me.

They also wrote i need no adaptations in new property even though i have adaptations here!

if I call them on monday could the report be removed from my file due to it being innacurate? if not what is likely to happen? and do i also call the housing association to tell them it's wrong or is it too late now they have a copy?

I was about to be offered a flat when it becomes available at the end of july and now I'm scared my HA will think there is nothing wrong with me and I don't need it (it has a wet room already and a few other adaptations)

Yes ask to speak to the OT’s manager, ask to be reassessed and if you get nowhere make a formal complaint. Tell the HA what you are doing.

I would contact both OT & HA. Ask if OT mixed up your report with someone else as it is inaccurate for your situation and needs to be amended.
Then contact HA & again say you believe they've submitted the wrong report as you were not asked the stated questions and it does not reflect your actual situation.
Highlight the inaccuracies to both in a follow up email so it is in writing and you have a dated paper trail if needed.

This is my personal opinion, I have no experience but I honestly think you need to get this information corrected as soon as possible.

Ugh, how anxiety provoking for you, OP. Agree with others, contact both the HA and the OT. Hope it’s sorted quickly.

The OT should have assessed you actually carrying out these activities rather than speculating on your abilities.
I seriously question the independence of some ot's.My elderly parent has had huge issues with an OT recently who removed some of her care with no meaningful assessment.The only avenue open to us has been to officially complain.

Yes definitely challenge it. I’ve just challenged a MH referral my GP sent that was totally fictitious, infuriating!

I’m not sure if this would apply to your situation but I disagreed with an NHS OT’s report done for DS earlier this year, sent a bullet point list of my concerns, and the OT updated the report immediately. Some things that she wrote were absolute rubbish and I was really cross but she updated it without any problem at all.
