Awful nappy rash, please help!

My 8mo DS has suddenly developed a really sore, angry nappy rash this week. He’s never had nappy rash before, and nothing has changed in terms of nappies, wipes, toiletries or laundry detergent. He’s been enjoying a variety of different foods recently, and I can only put it down to a combination of sweating in the hot weather and fruit acids in his poo making him so sore. We are scaling back his fruit intake to see if it helps. He has diagnosed CMPA and hasn’t had any allergic reactions to any other foods he’s had.

We change his nappy as soon as it’s wet or dirty so he is never left in a soiled nappy, he has nappy-free time every day and all the products we use are unscented. Currently using Sudocrem, and eking out the last dregs of a tube of yellow Metanium to cover the worst parts of the rash. We’ve also tried clotrimazole cream in case it’s fungal, but there hasn’t been much improvement. Taking him to a pharmacy tomorrow for advice, but please could you share your best nappy rash remedies as it’s so upsetting to see my poor baby crying at every nappy change because he’s so sore Sad would also appreciate any advice on foods that might cause or worsen nappy rash so we can limit/avoid them while he is healing. Thank you 🤞🏻

When my DD got a really bad nappy rash the pharmacist recommended cannesten thrush cream. You can only use it a couple of times a day if I remember correctly so we used bepanthem in between, but it cleared it up really quickly.

I found metanium dried her skin out too much and bepanthem was a lot more moisturising.

My DS is many years older now but suffered badly at times and I would always revert back to cotton wool and water to wipe and oilatum bath stuff for his bath each night and of course metanium which always helped which I know isn't available now. It did really help.

I think I possibly used talc to to very very light but understand people aren't always keen.

I have a bit of yellow metanium left in a used tube which I would gladly send to you...pm me if you would like it. It was the only thing that solved nappy rash for both my DC. I was going to say cut out acidic fruits but can see you're doing that. Wash bum with cotton wool (unbleached) and warm water only after nappy change, pat dry with soft towel and leave nappy off for as long as possible.

I had this with my youngest, I remember her devastating cries even now, I went through every cream possible, trailed around all the pharmacies for advice, they recommended the creams I'd already tried until one pharmacist told me to pop to Holland and Barrett (although I thought it odd I was desperate) they recommended any food grade coconut oil, as I said I was willing to try anything and blow me down from the first time I applied it I could see she had immediate relief, it cleared up within 2 days wouldn't have believed it if I hadn't seen it with my own eyes!

Weleda nappy cream only thing to work for us. Lots of natural ingredients and not harsh at all!

Flower0503 · 27/08/2025 22:41

Welda nappy cream worked with my CMPA baby. Could you go through what they have eaten past few days? Any recipe changes on products, have they swiped another kids biscuit or accidentally consumed milk in any way? Only suggesting as we have been caught out by these things before.

Cinnabonswirl · 27/08/2025 22:44

Cotton wool and water
dry it then bepanthen. I tried everything, we’d been to the drs, had all the anti fungal creams and everything else we could and then this cleared it up so quickly

As pp have said use cotton wool and water to clean. Pat dry. Leave out if nappies for as much of day as possible. Barrier cream overnight. It will take a few days to clear.

Get GP to check for thrush, which will need treatment.

Professionals will say there’s no connection but I would put money on him cutting his teeth. Happened with both mine and grandchildren. Weleda is very good. Sudocrem can burn on sore skin as it’s antiseptic. My friend swears by coconut oil.

Blessedbethefruitz · 27/08/2025 23:11

Sudosalve (can only use a few times a day as contains meds) is fantastic. Unless there might be an infection - our GP has prescribed daktakort once when ds was a baby, it was absolutely horrific for him. He had broken skin and little blood spots :(

I thought this too. OP, could it be because he is teething?

scaredfriend · 27/08/2025 23:38

Egg white. Paste it on raw and let it dry.

Helpmechooseausername · 27/08/2025 23:38

Another vote for teething! Mine all had horrific nappy rash when they were teething. I read that it's because they're salivating more which makes their poo more acidic, so it burns their bums.
Yellow metanium sorted it out when nothing else helped but I don't think it's available any more 😢

chilliheeler123 · 28/08/2025 11:39

Thanks everyone! I wonder if kiwi might be the culprit as he hasn’t had any for a couple of days and his bottom looks SO much better this morning. It’s a shame as he bloody loves kiwi, but it’s so acidic. Definitely no dairy cross-contamination as we’ve all gone completely dairy-free so we don’t have any dairy products at home (I can’t eat it anyway as still BF) and we make all his food ourselves. No more kiwis until he’s potty-trained!

I know teething can cause nappy rash, he has four teeth already and the rash started a couple of weeks after he cut his last two teeth so I don’t think it’s the cause in his case.

@Richtea67 that’s so kind of you to offer to help a complete stranger like that, but please keep hold of it in case there are any other little bums in your care that might need it someday! I’ve probably got enough to treat this, it looks empty but I’ve nicked some holes in the sides and there’s still a surprising amount coming out!

Just wanted to update for anyone else who’s battling nappy rash!

I have fixed it Smile combination of Cavilon barrier film and Egyptian Magic cream! I had some in the cupboard and wondered if it might work on his bum, and turns out it absolutely does. Happy little boy again Smile
