What is causing this lower back pain?

Pain in left lower back. There all the time. Heat helps it. Tried painkillers and nothing really helps. And obviously don’t want to be on them long term anyway. Had an MRI and nothing showed up. Blood tests are normal.
It can sometimes be painful and links to needing to go to the toilet for a poo. (Sorry if TMI).
Can anyone help? What could it be?

Could be to do with your pelvic floor? If it’s over active and locked on all the time it can cause pain. I’d see a decent women’s physio.

I agree. I have intermittent lower back pain, linked to falling off a horse many years ago. I have monthly deep massage, and see my chiropractor every 3 months (although it was more frequent until we got it under control)

You may also want to check your seating position, loads of back pain is caused by sitting slightly twisted. This tightens up the muscles on one side over time, causing pain. Then the body gets so used to that position that correcting it can be a real pain too. If you do go to a chiropractor they should be able to spot that.
