How does this work for Rail Travel?

So, I have purchased a digital rail card online which gives me a 1/3 discount on rail travel due to having a disability. However, I am unsure as to how exactly I would use it.

When I've looked at buying train tickets, the websites have asked whether I had some sort of discount and when I said yes, they reduced the price. However, they didn't ask for proof of my rail discount. So, how would they know I was eligible to a discount? Who would check?

If I travelled with a digital disabled persons' rail card, would the train staff ask to check my phone or something like that after seeing that my ticket was discounted - is that how it works?

Also, does this apply to all trains in the UK or specific trains?

You need to be able to show it on the train, when they check your ticket they ask to see the railcard.

Yes the conductor checks the railcard when they check your ticket.

Yes whoever checks your ticket on the train will ask to see your Railcard. You'll be charged for a new, full price ticket if you're unable to show it.
