That’s ridiculous… what’s the bloody point of making notes that nobody else can read!

I’d be asking for it to be rewritten, because what if it’s actually important?

I am struggling a little to get the photos to load well on my little phone.
Were you having some issues with your kidneys? Protein and creatinine levels etc?
Medical notes have all sorts of abbreviations- it is almost another language and won't be written in a way that non medical will necessarily understand.

Might be better to ask for clarification from your midwife who can explain the context? A few lines out of the book makes it a little hard to determine the situation

You were there because reviewed by someone else a few days ago and noted to have a large amount of protein in the urine. The plan was to do blood tests and await the results of urine/ plasma creatinine levels to look at your kidney function.

Thankyou everyone. As an x nurse I've been quite used to deciphering medical notes but struggled with these. These are in the hand held maternity notes.
I got the gist of most of it.
It's the last photo that is still a mystery.

Bloods - nil
Something UPCR

Then it iays she was reviewed by someone a few days ago. I think it says advised to come in for repeat UPCR in view of high levels of protein in urine

And then I think safety net advise given

Re referred by one of the team a few days ago to repeat urine analysis and UPCR (Urine Protein Creatinine Ratio) in view of +++ protein in her urine.

Urine Analysis (photo cuts off so don't know result). She is asymptomatic of PET (Pre eclampsia Toxaemia). Bloods normal, ?await UPCR

Safety net advice given.

No need for antihypertensives for now.
Basically concerned with protein in urine and want to repeat in order to monitor for pre-eclampsia.

This was a follow up appointment yesterday after spending Monday afternoon/evening in hospital with contractions and severe back ache..(36 weeks). She was given antibiotics for ? Urine infection and
her contractions stopped.
When she left after her appointment yesterday she was called back to say that her urine analysis results were back and showed ecoli infection so started new antibiotics..
She's feeling a lot better today..
