I'm struggling with keeping on top of everything

I know it's all connected. I'm constantly tired, I eat badly, have no energy for exercise, overweight, disorganised at work, house is a mess, mind is frazzled as I'm on the phone too much.
Some people might suggest adhd and that could be a possibility. I just need to get out if this hole I've found myself in.
Has anyone used any apps for organisation they could recommend?

The Cozi app is great. It puts your shopping list, calendar and to do list in one place which I find really useful.

I know it's easier said than done but I find it really useful writing down a meal plan for each week because it takes the thinking out mid week of what to cook. Tesco have a great meal plan creator that also produces a shopping list. Create meal plan | Tesco Real Food. If cooking each night doesn't appeal, I also try to make Slow Cooker Dump Bags {Freeze and Go Meals} when I have the time and energy. You can pull one out the freezer the night before, shove it in the slow cooker before work and come home to dinner.

On a separate note, if you're wanting to go down the ADHD diagnosis pathway then when you're speaking to your GP, say you want to go through the 'right to choose' pathway and mention a provider such as CARE ADHD. They've got much, much faster wait times and are still under the NHS (so still completely free), and are authorised to prescribe ADHD meds on the NHS also.

I've found since finding the money for a cleaner (by cutting back in other areas) my home is cleaner which makes me feel less suffocated by "stuff" and worry about the home. I really struggled with overwhelm before this, it's also forced me to tidy once a week which is bringing me into a better routine. Deleting social media has also helped my only doom scrolling now is here. I consciously put my phone down when my children want my attention and set it to DND when flipped on its front.

I also thought I might have ADHD but no, all of the above has fixed all that. It may or may not work for you but it's been the best thing I could have done.

Maybe what you need is not an app but to spend less time on the phone.
Read, go for walks, etc...

You already know what you need to change. Start small. Do a tiny bit here, a tiny bit there. Put your phone down for 5mins. Don't buy that chocolate bar. Drink that water. Go for a 10min walk. Soon enough a tiny bit will turn into a big bit. And you'll start feeling much better.

I schedule everything I need to go on google calendar. and make myself do whatever it is i'm supposed to do when the alert comes on.
